Advances in mechanical thrombectomy for acute ischaemic stroke from large vessel occlusions

Abstract: Advances in endovascular treatment of acute ischaemic stroke from intracranial large vessel occlusions have continued in the past decade. Here, we performed a detailed review of all the new trials and studies that had the highest evidence, the guidelines for mechanical thrombectomy, the selection of the particular population outside the guidelines and endovascular therapeutic strategies for acute ischemic stroke from occluded intracranial arteries.

“…Acute ischemic stroke (AIS) is a devastating cerebrovascular disease, and timely recanalization by intravascular thrombolysis and/or endovascular thrombectomy (EVT) is the primary therapeutic goal ( 1 ). Randomized controlled clinical trials and meta-analyses have established that EVT treatment for acute large vessel occlusion (LVO) within the proximal anterior circulation achieves >80% of near-complete or complete angiographic recanalization [modified thrombolysis in cerebral infarction score (mTICI) of 2b-3].…”

“…Large arterial occlusion (LAO) has become one of the most important causes of acute ischemic stroke (AIS) worldwide and is closely related to the poor prognosis of AIS patients ( 3 ). In the past 10 years, endovascular treatment of large-arterial occlusion acute ischemic stroke (LAO-AIS) has achieved good progress ( 4 ). For such patients, mechanical thrombectomy (MT) is one of the main treatment approaches.…”
