Scrotal surgery encompasses a wide-variety of surgical techniques for an even wider variety of indications. In this manuscript, we review our indications, techniques, and pit-falls for various reconstructive scrotal surgeries as-well-as surgical tips for placement of testicular prostheses. Penoscrotal webbing (PSW) is an abnormal, often-problematic distal insertion of scrotal skin onto the ventral penile shaft. There are several effective and straightforward techniques used to revise this condition, which include simple scrotoplasty, single- or double-Z-plasty, or the VY-flap scrotoplasty. Reconstruction is also commonly indicated following scrotal skin loss caused by infection, trauma, lymphedema, hidradenitis, and cancer. Although initial management of these conditions often involves scrotal skin removal, repair of expansive scrotal skin loss can be technically difficult and can be accomplished by using one of several skin flaps or skin grafting. Split-thickness skin grafting of scrotal defects can be accomplished easily, and provides durable results.

Introduction: Surgical site infection (SSI) following hydrocelectomy is relatively uncommon, but it is one of the main post-operative problems. We aimed to describe the prevalence of SSI following hydrocelectomy among adult patients, and to assess predisposing risk factors for infection. Methodology: This retrospective cohort study was carried out at a university hospital and included hydrocelectomies performed between January 2007 and December 2014. Diagnosis of SSI was performed according to the Center for Diseases Control (CDC) guidelines. Multivariable logistic regression analysis was used to identify independent risk factors. Results: A total of 196 patients were included in the analysis. Overall, 30 patients were diagnosed with SSI (15.3%) and of these, 63.3% (19/30) were classified as having superficial SSI, while 36.7% (11/30) had deep SSI. The main signs and symptoms of infection were the presence of surgical wound secretion (70%) and inflammatory superficial signs such as hyperemia, edema and pain (60%). Among the 53 patients presenting chronic smoking habits, 26.4% (14⁄53) developed SSI, which was associated with a higher risk for SSI (odds ratio [OR] = 2.84, 95% confidence interval [CI] = 1.27 to 6.35, p < 0.01) in the univariate analysis. In the adjusted multivariable analysis, smoking habits were also statistically associated with SSI after hydrocelectomy (odds ratio [OR] = 2.84, 95% confidence interval [CI] = 1.30 to 6.24, p = 0.01). No pre-, intra-, or post-operative variable analyzed showed an independent association to SSI following hydrocelectomy. Conclusions: Smoking was the only independent modifiable risk factor for SSI in the multivariate analysis.

Spermatocele is cystic dilatation of epididymis. These cysts are usually located outside of the tunica vaginalis and show transillumination as hydrocele. They are detected during a scrotal ultrasound incidentally. Although the etiology of most spermatoceles is idiopathic, trauma, infection or inflammatory process may lead to the development of spermatocele. Our case is a 69-year-old male patient with localized prostate cancer. He was admitted to our hospital with scrotal pain 3 months after open radical retropubic prostatectomy. His scrotal examination revealed 2 soft-density masses measuring 3-4 cm in diameter in the left scrotum and one measuring 5-6 cm in the right scrotum. Scrotal color Doppler ultrasonography showed two cystic masses filled with liquids one measuring 5.5 cm in the right scrotum and one measuring 2.4 cm in the left scrotum. Bilateral scrotal exploration was done and two spermatoceles, one measuring 3.4 cm in the left scrotum and the other measuring 5.6 cm in the right scrotum, were removed. Bu kistler genellikle tunika vajinalisin dışında olup hidroseller gibi transluminasyon verirler. Sıklıkla skrotal ultrasonografi sırasında insidental olarak saptanırlar. Birçok spermatosel idiyopatik olmakla birlikte travma, enfeksiyon veya enflamatuvar olaylar spermatosel gelişimine neden olabilirler. Olgumuz 69 yaşında erkek hasta lokalize prostat kanseri nedeniyle yapılan açık radikal retropubik prostatektomi sonrası post-op 3. ayda skrotal ağrı şikayetiyle kliniğimize başvurdu. Skrotal muayenesinde sağda 5-6 cm boyutlarında solda 3-4 cm boyutlarında yumuşak dansiteli kitle tespit edildi. Yapılan skrotal renkli doppler ultrasonografide sağ skrotumda içi sıvı dolu yaklaşık 5*5 cm ve solda 2*4 cm boyutlarında kistik kitle tespit edildi. Bilateral skrotal eksplorasyon yapıldı ve sağ skrotumdan yaklaşık 5*6 cm boyutlarında sol skrotumdan 3*4 cm boyutlarında spermatosel çıkarıldı. Sıklıkla epididim başında gelişmektedir. Etiyolojisi tam olarak aydınlatılamasa da yaşlanmayla birlikte kanalların aglütine germ hücreleri ve semen pıhtılarıyla tıkanmasından kaynaklandığı düşünülmektedir (1). Bu oluşumlar süt renginde olabilen ve spermatozoa içeren sıvıyla doludur. Tanıda skrotal ultrasonografi kullanılmakta ve hidrosel gibi translüminasyon vermektedirler. Bu kistler ağrısız, yuvarlak ve keskin sınırlıdır. Tedavi, kistin büyüklüğüne, semptomların ciddiyetine ve cerrahın tercihine bağlıdır; çok büyük ve ağrıya neden olmadıkları sürece izlenmelerinde sakınca yoktur. Aspirasyon ve skleroterapinin (kist içine ilaç verilmesi) tedavide yeri yoktur.
