Objective: Adiponectin is linked to a favorable lipoprotein profile, but potential longitudinal associations are not known. Design: A population-based follow-up study of all inhabitants born in 1942, 1947, 1952, and 1957 (nZ1294) in Pieksämäki, a town in Finland. Of the 690 subjects participating in both the check-ups, 228 subjects with diabetes or any medication for dyslipidemia, high blood pressure, or diabetes were excluded. The final study population consisted of 462 (182 men and 280 women) apparently healthy subjects. Methods: Main outcome measures were lipoprotein particle sizes and concentrations, apolipoprotein A-1 (APOA1) and APOB levels at baseline and follow-up across baseline adiponectin tertiles. Serum adiponectin concentrations were determined using an enzyme immunoassay, and lipoprotein subclasses using proton nuclear magnetic resonance spectroscopy. Results: At the second health check-up 6.4 years later, the very low-density lipoprotein particle concentration decreased across the baseline adiponectin tertiles in men from 1.04 (0.28) to 0.91 (0.29) nmol/l (P for linearityZ0.011) and in women from 0.92 (0.32) to 0.80 (0.24) nmol/l (PZ0.002). Correspondingly, the mean high-density lipoprotein particle size increased from 9.78 to 9.90 nm in men (P!0.006) and from 10.00 to 10.14 nm in women (P!0.001).

Plasma Adiponectin Concentration and Its Association with Metabolic Syndrome in Patients with Heart Failure

PurposePlasma adiponectin concentrations are inversely related with metabolic syndrome (MetS), and MetS is associated with increased risk for heart failure (HF). However, the relationship between adiponectin and MetS in HF remains undetermined. Therefore, we tested whether MetS was associated with the degree of plasma adiponectin concentrations in HF patients.Materials and MethodsOne hundred twenty eight ambulatory HF patients with left ventricular ejection fraction of <50% (80 males, 61.8±11.9 years old) were enrolled for this cross-sectional study. Echocardiographic measurements were performed, and plasma concentrations of adiponectin, lipoproteins, apolipoproteins (apoB, apoA1) and high sensitive C-reactive protein (hsCRP) were measured.ResultsAdiponectin concentrations in HF patients with MetS (n=43) were significantly lower than those without MetS (n=85) (9.7±7.0 vs. 15.8±10.9 µg/mL, p=0.001). Higher concentrations of apoB (p=0.017), apoB/A1 ratio (p<0.001), blood urea nitrogen (p=0.034), creatinine (p=0.003), and fasting insulin (p=0.004) were observed in HF patients with MetS compared with those without MetS. In HF patients with MetS, adiponectin concentrations were negatively correlated with hsCRP (r=-0.388, p=0.015) and positively correlated with the ratio of early mitral inflow velocity to early diastolic mitral annular velocity, E/E' (r=0.399, p=0.015). There was a significant trend towards decreased adiponectin concentrations with an increasing number of components of MetS (p for trend=0.012).ConclusionOur study demonstrated that adiponectin concentrations decreased in HF patients with MetS, and that relationship between adiponectin, inflammation and abnormal diastolic function, possibly leading to the progression of HF.

Clinical interpretation of patients' plasma adiponectin (APN) remains challenging; its value as biomarker in dilated cardiomyopathy (DCM) is equivocal. We evaluated whether circulating APN level is an independent predictor of composite outcome: death, left ventricle assist device (LVAD) implantation, and heart transplantation (HT) in patients with nonischemic DCM. 57 patients with nonischemic DCM (average LV diastolic diameter 6.85 cm, LV ejection fraction 26.63%, and pulmonary capillary wedge pressure 22.06 mmHg) were enrolled. Patients underwent echocardiography, right heart catheterization, and endomyocardial biopsy. During a mean follow-up of 33.42 months, 15 (26%) patients died, 12 (21%) patients underwent HT, and 8 (14%) patients were implanted with LVAD. APN level was significantly higher in patients who experienced study endpoints (23.4 versus 10.9 ug/ml, p = 0.01). APN was associated with worse outcome in univariate Cox proportional hazards model (HR 1.04, CI 1.02–1.07, p = 0.001) but lost significance adjusting for other covariates. Average global strain (AGS) is an independent outcome predictor (HR 1.42, CI 1.081–1.866, p = 0.012). Increased circulating APN level was associated with higher mortality and may be an additive prognostic marker in DCM with advanced HF. Combination of serum (APN, BNP, TNF-α) and echocardiographic (AGS) markers may increase the HF predicting power for the nonischemic DCM patients.
