Adenocarcinoma of the bladder following nephrogenic adenoma: a case report

Abstract: Introduction: Nephrogenic adenomas are generally considered to be benign lesions, but there remains a risk for malignant transformation. Patients with immunosuppression in particular appear to be at a higher risk of malignant disease. We report a case of post-traumatic nephrogenic adenoma in a young patient without immunosuppression, which transformed into an adenocarcinoma of the bladder.

“…2 However, approximately 10% of the lesions are greater Hospital Universitario Central de Asturias, Oviedo, Spain than 4 cm in diameter, as in the case reported. 8 Upon immunohistochemical analysis, nephrogenic adenomas are positive for the renal tubule markers CK7, CD10, AMACR, PAX 2, and PAX 8 but negative for bladder urothelium antigens such as p63. 2 The transcription factors PAX 2 and PAX 8 are expressed in nearly 100% of nephrogenic adenomas in some series.…”

“…Clinical symptoms, when present, are non-specific such as lower urinary tract symptoms and hematuria [ 22 , 23 ]. Therapeutic options vary from watchful waiting to cystectomy [ 23 ], whereas in most cases a transurethral resection is favored [ 24 ], as it was the case in our 3 patients with a nephrogenic adenoma. A high recurrence rate up to 80% has been reported [ 23 ].…”

“…A high recurrence rate up to 80% has been reported [ 23 ]. Although nephrogenic adenoma is usually thought to be a benign entity [ 22 ], there might be some potential for malignant transformation as its occurrence has been reported in association with transitional cell carcinoma [ 23 ] and moderately differentiated adenocarcinoma of the bladder in a patient with NLUTD [ 24 ]. Because of the high recurrence rate of the nephrogenic adenoma and its assumed potential for neoplastic transformation, regular follow-up should be considered.…”

“…Although development of malignancy from NA in adults has been reported, no case of malignant transformation has been reported in pediatric patients. [10] The presentation of NA is usually with hematuria and lower urinary tract symptoms of frequency, nocturia and urgency. [1] Cystoscopy provides the initial visual diagnosis in suspicion of transitional cancer.…”
