Adapting PCIT to address mental health care disparities among underserved families impacted by pediatric illness: A case series of bedside PCIT.

Abstract: Objective: Medically ill children are at elevated risk for mental health issues (Blackman, Gurka, Gurka, & Oliver, 2011), which can negatively affect health status (DeMore, Adams, Wilson, & Hogan, 2005; Johnston & Martin-Herz, 2010). Given that children from diverse racial/ethnic backgrounds are disproportionately impacted by medical conditions (Mehta, Lee, & Ylitalo, 2013) and experience marked mental health care disparities (Marrast, Himmelstein, & Woolhandler, 2016), innovative interventions are necessary f… Show more

“…Individual interventions, such as Parent Child Interaction Therapy (PCIT), can also be adapted to telehealth for children with chronic medical conditions (Christian-Brandt & Santacrose, 2020; McQuaid & Everhart, 2020). One such case study is presented in the current issue, in which PCIT was delivered via telehealth for a family of a patient with cancer during the beginning of the COVID-19 pandemic (Bono, 2022).…”

“…[16,17] Traditional PCIT was developed as a clinic-based program for the treatment of disruptive, externalizing behavior disorders in children aged 3 to 6 years old. [18] A limited number of prior case reports [19][20][21][22][23] and 1 case series [24] have provided support for the application of PCIT in chronically ill or medically complex children. These reports outlined treatment courses where PCIT was either performed within outpatient mental health settings (the setting that PCIT was originally designed for) or adapted to the hospital bedside.…”

“…One key factor in pediatric health disparities continues to be variability in access to care (Larson, Cull, Racine, & Olson, 2016). Articles by Crosby, Strong, Johnson, and Mitchell (2020); Wihak et al (2020), and Christian-Brandt and Santacrose (2020) provide innovative strategies to increase access and deliver evidence-based care to diverse populations. Two of these articles (Crosby et al, 2020; Wihak et al, 2020) focus specifically on sickle cell disease (SCD).…”

“…Finally, Christian-Brandt and Santacrose (2020) describe a case series that illustrates an adaptation of another evidence-based treatment, parent-child interaction therapy, for use with medically ill children. Similar in concept to the work of Wihak et al (2020), this approach represents a strategy to deliver an evidence-based treatment to underserved children who might not otherwise be able to access behavioral health services (Marrast, Himmelstein, & Woolhandler, 2016).…”
