Aconitum, also known as monkshood, wolfʼs bane, or devilʼs helmet, has been widely used in folk medicine in China, India, and certain parts of Europe [1-4]. The genus Aconitum (Ranunculaceae) comprises 300 species distributed all over the world [5]. The most common species are Aconitum carmichaelii Deb. and Aconitum kusnezoffii Rchb. in China, Aconitum japonicum Thunb. in Japan, Aconitum napellus L. in Europe, Aconitum ferox Wall. ex Ser. in India, and Aconitum noveboracense A. Gray ex Coville in the United States [5]. Several classes of secondary metabolites, especially alkaloids, have been isolated from different Aconitum sp. [6]. The type of the isolated major alkaloids may vary depending on the species such as aconitine, hypaconitine, and mesaconitine from A. carmichaelii, aconitine from A. napellus, hypaconitine from A. septentrionale Koelle, mesaconitine from A. kusnezoffii, bikhaconitine from A. ferox, talatisamine from A. kongboense Lauener, atisine from A. anthora L., and A. heterophyllum Wall. ex Royle, and lycaconitine from A. vulparia Rchb. [6]. Several isoquinoline alkaloids and phenethylamine derivatives have also been isolated, such as higenamine from A. japonicum, magnoflorine from A. vulparia and A. napellus, coryneine from A. carmichaelii, and N-methyl adrenaline from A. nasutum Fisch. ex Rchb. [6]. Lipo-alkaloids including lipoaconitines, lipomesaconitines, lipodeoxyaconitines, and lipohypaconitines were also isolated [7, 8]. The efficacy of Aconitum sp. in resolving critical clinical conditions has been proven by doctors practicing traditional Chinese medicine (TCM) and Ayurvedic medicine for centuries. However, the long history of Aconitum sp. misuse in homicide cases has shaken the faith in the potential safe application of this herb in therapy [9, 10]. The recent developments in analytical techniques which can identify and determine the concentrations of toxic compounds in herbal products with impressive accuracy and reliability have rekindled the interest in Aconitum preparations [1, 5, 6, 11, 12].

Background: Goshajinkigan (GJG) is used for the treatment of several neurological symptoms. We investigated the efficacy of GJG and mecobalamin (B12) against neurotoxicity associated with docetaxel (DOC) in breast cancer patients. Materials and Methods: Sixty breast cancer patients were treated with DOC. Thirty-three patients (GJG group) received oral administration of 7.5 g/day GJG and 27 patients (B12 group) received oral administration of 1500 μg/day B12. Neuropathy was evaluated according to DEB-NTC (Neurotoxicity Criteria of Debiopharm), Common Terminology Criteria for Adverse Events (NCI-CTC) ver. 3.0, and a visual analogue scale (VAS). This study employed a randomized open design. Results: The incidence of neuropathy was 39.3% in the GJG group, and 88.9% in the B12 group (p<0.01). In the GJG group, grade 1 DEB-NTC was observed in 2 cases, grade 2 in 5 cases and grade 3 in 5 cases. Grade 1 NCI-CTC was observed in 7 cases, grade 2 in 6 cases, and VAS was 2.7±2.2. In the B12 group, grades 1, 2 and 3 DEB-NTC were observed in one case, 12 cases and 12 cases, respectively; and grades 1, 2 and 3 NCI-CTC were observed in 11 cases, 12 cases and one case, and VAS was 4.9±2.4. Conclusions: Concomitant administration of GJG is useful in preventing neuropathy in breast cancer patients treated with a DOC regimen.

Objectives Certain symptoms and signs are culturally specific. 'Hie' (chill sensation) is a major symptom experienced by Japanese people; however, it is not easily understood by Westerners. Although Hie is not lifethreatening, it greatly hampers the quality of life in sufferers. To develop a remedy for Hie, valid and reliable measures are required. This is the first study aimed at developing a standardized questionnaire to quantitatively measure Hie symptom. Methods This was a cross-sectional study. To identify question items, we conducted a literature search using published books that mention Hie and related symptoms. The first draft of the questionnaire was prepared by selecting 31 items, including three empirically used items, using the Delphi method. A total of 744 Japanese volunteers completed the draft questionnaire. Simple correlation and factor analyses were performed to select items for the final version of Hie questionnaire and for evaluating its test-retest reliability. Results The following ten question items were ultimately selected: feeling a breeze, shivery feeling, tolerance, sensitivity to cold, Hie-like sensation in an airplane, dislike of air conditioning, use of gloves, use of an electric blanket, use of heavy clothing and need for heating devices. Of the ten Hie-related question items, five pertained to physical symptoms and the other five to daily behaviours. The internal consistency of the ten-item questionnaire was high, with a Cronbach's alpha of 0.85. The test-retest reliability of the questionnaire was preserved by the paired two-tailed t test. Conclusions A new questionnaire was developed to evaluate the subjective symptom of Hie. This questionnaire demonstrated sufficient reliability and could be used as a tool to assess this symptom.
