A rare presentation of primary cutaneous basaloid squamous cell carcinoma of scalp

Abstract: Basaloid squamous cell carcinoma (BSSC) is a rare and aggressive variant of squamous cell carcinoma (SCC). It has predilection for the upper aero digestive tract with common metastasis to regional lymph node and common distant metastasis to lungs. While metastasis to scalp has rarely been reported, primary occurrence of BSCC arising from scalp has hardly ever been reported. We are reporting a case of 70 year female patient, who presented with an ulceroproliferative growth in posterior scalp. Biopsy from the ed… Show more

“…Primary cutaneous BSCC is a rare clinical entity with seven other cases reported and none of which resulted in brain metastasis. [2][3][4] BSCC portends a worse prognosis given the high propensity for lymph node and distal metastasis. 1 BSCC shows a strong correlation with oncogenic HPV strains, especially HPV16 in oropharyngeal and anogenital areas.…”

“…1 To date, seven cases of primary cutaneous BSCC outside the upper aerodigestive tract and mucosal genital regions have been reported. [2][3][4] Having BSCC portends a worse prognosis, as up to 75% of patients have advanced nodal or metastatic disease. 3 BSCC can metastasize to any organ, but brain metastasis rarely occurs.…”

“…In contrast to BCC, SCC is typically positive for cytokeratins and EMA but negative for BerEp4 and Bcl2. [18][19][20] Histologic features of poorly differentiated SCC include immature cells, multiple mitotic figures, and scarce keratinization. P63 is highly sensitive in this diagnosis, 21,22 but this marker is also positive in BCC.…”
