rESUMo objetivo: O objetivo deste trabalho foi fazer uma revisão sistemática da literatura, utilizando a base de dados MedLine, sobre o tema: uso/abuso/dependência de álcool como um fator de risco à redução da adesão, à redução na supressão da carga viral ou ao pior desfecho clínico em pacientes com AIDS em uso de highly active antiretroviral therapy (HAART). Método: Foi realizada uma pesquisa sistemática na base de dados MedLine utilizando como unitermos "HAART", "adherence" e "alcohol", na busca de artigos que versassem sobre a temática: avaliação ou associação de uso/abuso/dependência de álcool e adesão/supressão da carga viral/ desfecho clínico nos pacientes em uso de terapia antirretroviral. resultados: A busca resultou em 65 artigos. Contudo, apenas 21 deles contemplaram os critérios de inclusão e foram selecionados. Foi encontrada associação positiva entre uso/abuso/dependência de álcool e baixa adesão/baixa supressão da carga viral/pior desfecho clínico em 18 (85,7%) artigos. Conclusão: O uso/abuso/dependência de álcool é um fator de risco para baixa adesão/baixa supressão da carga viral/pior desfecho clínico nos indivíduos em uso de HAART. aBStraCt Objective: The purpose of this work was to make a systematic review of the literature, using the MedLine data basis, about the alcohol usage/abuse/dependence as a risk factor to the adherence reduction, the viral load reduction or even a worse clinical outcome in HIV patients in the usage of highly active antiretroviral therapy (HAART). The impact of medication regimen factors on adherence to chronic treatment: a review of literature

This article reviews recent literature in chronic illness or long-term health management including asthma, contraception, diabetes, HIV disease, and hypertension/cardiovascular disease, mental disorders, pain, and other diseases to determine the relationship between regimen factors and adherence to medications. The authors conducted an electronic literature search to detect articles published between 1998 and 2007. Articles were included if they pertained to a chronic illness or to contraception, included a clear definition of how adherence was measured, and included regimen factors as primary or secondary explanatory variables. Methodology of the studies varied greatly, as did methods of measuring adherence and regimen factors. Surprisingly few of these articles concerned (1) chronic treatment, (2) regimen factors such as dosing, pill burden, and regimen complexity, and (3) adherence measured in a clear manner. Most studies failed to use state-of-theart methods of measuring adherence. Despite these flaws, a suggestive pattern of the importance of regimen factors, specifically dose frequency and regimen complexity, emerged from this review.

Background: Adherence to antiretroviral therapy (ART) is the strongest indicator of successful treatment of Human Immunodeficiency Virus (HIV) among children. The main aim of the present study was to know the prevalence of adherence at our centre and various factors affecting the adherence in children.Methods: It was an observational study done in children less than 15 years of age, affected with HIV. 78 children attending Paediatric Centre of Excellence (PCOE) for HIV L.T.M.G. Hospital, Sion, Mumbai were included in study, Study period was 18 months from January 2012 to June 2013. Complete history of the patients was noted in a pre-designed proforma. Baseline investigations related to HIV were done in all children. Fixed dose combination ART was started in children who fulfilled clinical and/or immunological criteria as per the NACO guidelines. Adherence was estimated using Pill count method..All the data were analysed by using 10.0 version of statistical software SPSS.Results: In this study, male children (57.7%) out numbered the females (42.3%). Majority of study subjects were more than 10 years of age. Overall adherence among subjects was 82.1% at the end of one year and non-adherence was 17.9%. There was no significant association of age and gender of the patients with adherence (p>0.05). Education, employment and knowledge towards medication of care takers was significantly associated with adherence of ART (p<0.05). Adherence of study cases had not showed any significant association with ART treatment regimen (p=0.99). At the end of one year, the mean CD4 count was significantly more as compared to baseline among adherence group and the mean CD4 count was significantly less as compared to baseline among non-adherence group. The most common reason for missed dosage was forgot to take medicine in 29 subjects (37.1 %)..Conclusion: Adherence level in this study was good. Forgot to take the medicine was the main reason given by the patients for non-adherence. Education, employment and knowledge towards medication of care takers was significantly associated with adherence of ART .But further studies are needed to explore various other factors related to adherence in children.

Abstract: Nearly all patients' adherence levels were suboptimal, demonstrating the critical need for programs to assist patients with medication taking. Interventions that assess and treat substance abuse and incorporate adherence aids may be particularly helpful and warrant further study.
