A Proper Sequence for the Treatment of B16 Melanoma: Chemotherapy, Surgery, and Immunotherapy

Abstract: The therapeutic effectiveness of surgery, chemotherapy, and immunotherapy alone and in combination were studied in the B16 melanoma model. The sequence of chemotherapy and immunotherapy as adjuvants to surgery proved important: Chemotherapy was significantly better when given before surgery; immunotherapy was more effective when delivered after surgery. The most effective therapeutic regimen was a combination of all three modalities: a single course of chemotherapy preceding surgery followed by immunotherapy.

“…The nonspecific immunostimulants such as BCG, C. parvum have been efficacious even for clinical tumors such as lung cancer, melanoma, etc. [7,8], However, it is still controversial whether nonspecific immunotherapy in addition to cytoreductive chemotherapy is effective or not for advanced solid tumor [9], and this important question should be studied more in detail by using an appropriate solid animal tumor model. In the field of leukemia immunochemotherapy, Murphy and LoBuglio [10] reported that guinea pig leukemia may represent a useful model for exploring the effect of immunomodulation.…”

“…There are experimental animal data and theoretic considerations to support the use of preoperative (neoadjuvant) chemotherapy [29]. Experimental tumors are most effectively eradicated when chemotherapy is given before surgery.…”

“…From rapidly accumulating knowledge we hope to be able to construct a rational therapy of the future which will include surgery, chemotherapy, and immunotherapy (Pendergrast et al 1976, Al-Sarraf & Costanzi 1978, Kaufman et al 1978, Oettgen 1977, Old 1977 in a successful multimodality attack on metastatie malignant melanoma.…”
