A new molecular multiplex IgE assay for the diagnosis of pollen allergy in Mediterranean countries: A validation study

Abstract: Sensitivity and specificity of ESEP in testing serum IgE antibodies against pollen allergen extracts and molecules, in Italian patients with SAR, both exceeded 85%. The advantages and limitations of a multiplex customized immunoblot assay, in the routine clinical use of molecular diagnostics in Southern European pollen allergic patients, deserve to be tested.

“…53 There are several commercially available immuno-solid-phase multiplex allergen arrays: the Thermo Fisher ImmunoCAP ISAC (Immuno-solidphase Allergen Chip), which contains 112 allergens from 51 allergen sources 12 ; the new ImmunoCAP ISAC 112i, with 112 components from 48 allergen sources, where some components have been removed (Hymenoptera components, Pla a 2, Jug r 2) and some others were added (Ana o 3, Can f 4, Can f 6, Cor a 14, Der p 23 and alpha-Gal); the MADx Allergen Explorer (ALEX; containing 282 allergens: 156 extracts and 126 components) 54 ; and the Euroline microstrips. 55 The large number of extracts/ allergens from multiple classes of allergenic sources (Table 2) provides extensive and detailed information about a patient's sensitization profile. 54,56 Other diagnostic tools based on allergen arrays are under development, and new tools will likely be available in the near future.…”

“…Substantial agreement between MAST and ImmunoCAP was found for inhalant, food, and venom allergens, and it thus represents a valid diagnostic alternative. 55 Finally, epitope mapping will be the real future, provided that a large number of clinical and experimental data become available. Indeed, peptides are recognized by humans in an HLArestricted manner.…”

“…Children (10-18 years) were enrolled in the Department of Pediatrics of "Sandro Pertini" Hospital in Rome (Italy), and adults (19-60 years) were enrolled in the Hospital "S.Maria degli Angeli" of Pordenone (Italy). 27 The recruited patients underwent interviews with standardized questionnaires, skin prick test (SPT), and blood drawing, followed by a second visit, including NS collection. A subset of patients underwent a second blood drawing, performed the same day as the NS sampling (for further details, see Methods in Appendix S1).…”

“…27 Hence, polysensitization to many different pollen sources is typical in patients with SAR living in these areas. 27 This characteristic emerged also in our study population. The repertoire of allergens recognized by IgE in the patients' secretions at a molecular ( Figure 1) and source level ( Figure 2) confirmed a predominance of airborne-specific molecules.…”
