Intake of Polydextrose Alters Hematology and the Profile of Short Chain Fatty Acids in Partially Gastrectomized Rats

Polydextrose (PDX) ingestion may increase the intestinal absorption of iron. This study evaluated the effects of 7.5% polydextrose supplementation on markers of iron uptake, transport and storage in partially gastrectomized rats. Half of a batch of 40 male Wistar rats (250 g) underwent Billroth II partial gastrectomy with anterior truncal vagotomy (GXT), while the other half underwent sham gastrectomy (SHAM). At 7 postoperative days, the animals were subdivided into four groups (n = 10): Sham Control and GXT Control (no polydextrose); Sham PDX and GXT PDX (with 7.5% PDX). The animals were euthanized after 60 day of PDX treatment. Organ weight, cecal pH, the characterization and quantification of short-chain fatty acids (SCFA), hematological parameters, hepatic iron content and the expression of ferroportin (FPT) in the jejunum, cecum, colon and liver were evaluated. PDX caused changes in the cecum of the supplemented animals, where there was a decrease in pH, increase in cecal wall and marked production of SCFA, especially acetic and propionic acids (p < 0.05). Hepatic iron levels were lower in GXT animals. PDX increased hemoglobin (HGB) values by 29.2% and hematocrit (HCT) by 55.8% in the GXT PDX group compared to the GXT Control group. The GXT PDX group had lower hepatic FPT expression (p < 0.05). PDX led to increased SCFA concentration in the supplemented animals. Considering that SCFAs play a central role in the increasing nutrients uptake, this mechanism may be involved in altering the hematology profile observed in these animals but not enough to reverse iron deficiency anemia in post-gastrectomy rats.

Associations between Lifestyle Factors and Iron Overload in Korean Adults

It has been suggested that iron overload, which indicates the accumulation of iron, generates cellular reactive oxygens and causes peroxide damages to the body. Such oxidative stresses, in a broader context, are also caused by lifestyles such as alcohol consumption and smoking. However, there are limited data on the association between these lifestyle factors and internal iron overload. In present study, we evaluated associations between lifestyle factors, such as smoking status, alcohol consumption, and physical activity, and serum markers of iron overload. In a population-based cross-sectional study including 2,347 Korean men and women aged 49–79 years, we assessed serum transferrin saturation (TSAT) levels and defined iron overload as TSAT levels > 50% for men and > 45% for women. After excluding persons with chronic diseases and iron deficiency, multivariate odds ratio (OR) and its 95% confidence interval (CI) were estimated to evaluate associations between lifestyle factors and iron overload in 1,973 participants. In all participants, we examined a significantly positive association between heavy alcohol consumption (> 30 g/day) and iron overload; heavy drinkers showed 1.6-fold higher OR (95% CI, 1.11–2.36) than non-drinkers. Stratified analysis by sex showed that this association was significant only among men. In addition, we observed a potential association between heavy smoking > 10 cigarettes/day and iron overload (p = 0.07). In stratified analysis by sex, we examined a significant association between smoking and iron overload only among women; former or current smokers had 1.9-fold higher OR (95% CI, 1.01–3.63) than never-smoker. Our findings suggest that heavy alcohol consumption and smoking may worsen iron accumulation in the body.

Características Da Hemocromatose: Uma Revisão Narrativa

Objetivou-se conhecer, na literatura existente, as características da hemocromatose. O estudo é uma Revisão Narrativa de Literatura, realizada em setembro de 2013, nas bases Medical Line (MEDLINE), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), portal da Scientific Eletronic Library Online (SCIELO) e base de dados ScienceDirect, com os termos “hemocromatose” e “características” utilizadas no idioma português, sem recorte temporal. A busca inicial obteve 39 produções e a partir dos critérios de inclusão e exclusão, esta revisão elegeu 16 artigos. Os resultados possibilitaram o entendimento de como ocorre o processo da hemocromatose, caracterizando-a em hereditária e adquirida. Também, foi possível conhecer a associação desta com outras doenças e sua relação com etnias. Constatou-se que a hemocromatose é caracterizada pela sobrecarga orgânica de ferro, sendo mais frequente em caucasoides. Pode ser classificada em hereditária ou primária e adquirida ou secundária. A hereditária é uma doença autossômica recessiva, geralmente associada a mutações no gene HFE, sendo C282Y e H63D as mais frequentes. A hemocromatose secundária está associada a doenças congênitas ou adquiridas, onde a anemia hemolítica ou a eritropoese ineficaz requerem múltiplas transfusões sanguíneas. A hemocromatose também está associada à ingestão excessiva de ferro, ao alcoolismo e a doença hepática crônica. É preciso alertar a população, principalmente a vulnerável, sobre as características da doença e a importância do tratamento precoce.Descritores: Hemocromatose; Sobrecarga de Ferro; Transferrina.
