Background/Aim: Video-assisted thoracic surgery (VATS) for ipsilateral reoperations is challenging because of the potential for pleural adhesions. Insertion of the initial port can lead to lung injury because of the blinded intrathoracic area. We assessed the usefulness of ultrasonography before VATS to reduce the incidence of lung injury at the time of the initial port insertion during secondary ipsilateral VATS. Patients and Methods: Thirty-three patients who underwent repeat VATS for ipsilateral pulmonary lesions were included. All patients underwent preoperative ultrasonography to assess the possible presence of pleural adhesions using the lung sliding sign. Results: Seven adhesions were found at the VATS ports. Two of these adhesions were not evaluated as pleural adhesions using ultrasonography; however, they were loose. All initial ports were inserted without lung injury. There were no major complications. Conclusion: Preoperative detection of pleural adhesions using ultrasonography can determine the best initial port for secondary ipsilateral VATS. Patients and Methods Among 547 patients who underwent thoracic surgery at Nara Medical University Hospital from January 2017 to May 2019, 33 who underwent consecutive VATS for reoperations for ipsilateral pulmonary lesions were enrolled. All 33 patients underwent preoperative ultrasonography to assess the possible presence of pleural adhesions.

Objectives - to illustrate the clinical follow-up of a patient with chronic recurrent pericarditis subjected to a pericardial fenestration performed in a new manner, with subxiphoidal pericardiotomy, pericardioscopy followed by trans-pericardial thoracoscopy. Material and methods. A 69-year-old woman with chronic recurrent pericarditis and the threat of cardiac tamponade was subjected to subxiphoidal pericardiotomy, pericardioscopy and subsequent transpericardial thoracoscopy forming the pericardial window and junctions with pleural and abdominal cavity. Results. The first special aspect of the presented case is the combination of access types and the usage of non-standard access to the pleural cavity; the second one is the combined formation of fistulae between the pericardial cavity and two other cavities - the left pleural and abdominal. The positive outcome of this operation was the bidirectional drainage of pericardial exudate, as well as the minimal postoperative pain syndrome due to the absence of transthoracic access. Conclusion. The positive clinical result in the early postoperative period and during the long term followup period, the absence of complications allows for the safety of the technique and the possibility of performing such surgical operations in patients with acute and chronic exudative pericarditis and the threat of cardiac tamponade.

Introducción: Uno de los mayores avances de las últimas décadas en la cirugía de tórax ha sido el desarrollo de la cirugía mínimamente invasiva.Objetivos: Describir la experiencia en videotoracoscopía (VATS) uniportal de miembros del equipo de Cirugía de Tórax de la Universidad de Chile en 2 campos clínicos, (Clínica Las Condes y Hospital Clínico de la Universidad de Chile).Material y métodos: Se estudiaron 105 pacientes sometidos a VATS uniportal entre enero de 2016 y enero de 2019. Los datos se analizaron de manera retrospectiva considerando variables demográficas (edad, sexo) y clínicas (diagnóstico, cirugía, estadía hospitalaria, días de pleurostomía, conversión, y complicaciones). Resultados: De las 105 cirugías realizadas, 28 (26,6 %) correspondieron a cirugías mayores complejas lobectomías y segmentectomías anatómicas. En 4 pacientes se agregó un 2º puerto, uno se convirtió a mini toracotomía y uno a toracotomía (5,7% conversión global). La estadía hospitalaria fue en promedio 3,07 ± 3,1 días y el promedio de mantención de pleurostomía de 2,67 ± 1,61 días. Siete pacientes (6,6%) presentaron complicaciones post operatorias. Un paciente falleció por progresión de su enfermedad, no hubo mortalidad relacionada a la cirugía.Discusión: Las contraindicaciones de la VATS uniportal son las mismas que en la VATS multipuerto. En manos experimentadas, las complicaciones en cirugía por puerto único son bajas. Impresiona tener menos dolor post operatorio, menor estadía hospitalaria y reintegración precoz a las actividades diarias comparado con la VATS tradicional.Conclusiones: Se presenta la primera serie de VATS uniportal publicada en Chile. Los resultados obtenidos son comparables a los observados en la literatura. Su implementación y desarrollo requiere de una curva de aprendizaje similar a cualquier nueva técnica quirúrgica.
