Материал и методы. Проведен анализ лечения 2 пациентов с постстернотомным медиастинитом. После дебридмента раны в обоих случаях выполнена реконструктивно-восстановительная операция, включающая применение большого сальника и больших грудных мышц в первом случае и сочетание большой грудной мышцы и прямой мышцы живота во втором. Результаты. В обоих клинических наблюдениях послеоперационный период протекал без особенностей. Заживление раны проходило первичным натяжением. Пациенты выписаны с выздоровлением. Отмечен хороший отдаленный результат через 6 и 12 мес. Заключение. Особенности представленных клинических наблюдений -наличие хронической стернальной инфекции, представленной свищевым ходом с экссудацией. Ограниченность процесса и небольшой уровень микробной флоры позволили выполнить дебридмент раны одновременно с реконструктивно-восстановительной операцией. Оптимальный материал для реконструкции грудной стенки при постстернотомном медиастините -большой сальник и мышечные лоскуты (большая грудная и прямая мышцы живота). Финансирование. Aim to analyze the current possibilities of polypositional radiography and computed tomography (CT) in the diagnosis of infectious and inflammatory processes of the chest wall.
Material and methods. 221 patient with an infectious process in the sternum and ribs was treated in the surgical thoracic department of the "Samara Regional Clinical Hospital n.a. V.D. Seredavin" in the period from 2012 to 2020. All patients underwent CT of the chest as the primary diagnostic radiological method.
Results. Plain radiography allows for visualizing and evaluating the number of metal ligatures on the sternum, their position and relationship with it. The preference for CT is due to its capabilities both as a screening method and as a method for clarifying diagnostics (diagnosis verification). It helps to visualize the comparison of the sternal parts, the state of metal ligatures, determine the indications for bone resection and removal of incompetent fixing elements.
Conclusion. Each radiographic method for examining the chest has its own indications and its place in the arsenal of a thoracic surgeon.
Objectives - to illustrate the clinical follow-up of a patient with chronic recurrent pericarditis subjected to a pericardial fenestration performed in a new manner, with subxiphoidal pericardiotomy, pericardioscopy followed by trans-pericardial thoracoscopy. Material and methods. A 69-year-old woman with chronic recurrent pericarditis and the threat of cardiac tamponade was subjected to subxiphoidal pericardiotomy, pericardioscopy and subsequent transpericardial thoracoscopy forming the pericardial window and junctions with pleural and abdominal cavity. Results. The first special aspect of the presented case is the combination of access types and the usage of non-standard access to the pleural cavity; the second one is the combined formation of fistulae between the pericardial cavity and two other cavities - the left pleural and abdominal. The positive outcome of this operation was the bidirectional drainage of pericardial exudate, as well as the minimal postoperative pain syndrome due to the absence of transthoracic access. Conclusion. The positive clinical result in the early postoperative period and during the long term followup period, the absence of complications allows for the safety of the technique and the possibility of performing such surgical operations in patients with acute and chronic exudative pericarditis and the threat of cardiac tamponade.
Представлена история заболевания пациентки 71 года, перенесшей стернотомию и протезирование аортального клапана, осложнившегося развитием постстернотомного медиастинита. Отмечен хороший результат лечения с помощью вакуум-ассистированных повязок на этапе очищения раны с последующей пластикой передней грудной стенки кожно-подкожно-фасциально-мышечным лоскутом на верхней надчревной артерии.

The case report of a patient with post-sternotomy mediastinitis is presented. A successful case of treatment of such a formidable complication after cardiac surgery was demonstrated. Success in the treatment of such patients depends on the complete cupping inflammatory process in the site of operation, as well as chest wall reconstruction. The positive clinical effect of vacuum-assisted dressings in the treatment of post-sternotomy mediastinitis is shown. The vertical rectus abdominis myocutaneous flap was used as a plastic material to cover a defect in the chest wall. The course of surgery and the result of treatment are described in details. To date, the surgical society has not developed a generally accepted tactic in the treatment of poststernotomy mediastinitis, both at the stage of arresting the infectious process, and in the process of reconstructive and restorative intervention. In practice, along with alloplastic materials, synthetic and metal implants are used. The autologous tissues include muscle flaps and the greater omentum. In thoracic surgery, the rectus abdominis can serve as an alternative to omentoplasty for extended chest wall defects.
