A Case–control Study of Single‐Incision Versus Standard Laparoscopic Cholecystectomy

Abstract: There was no statistically significant difference in postoperative pain, pain site and analgesia requirement; however, patients who underwent SILC returned to their normal activity 1.8 days earlier than the LC patients. Larger RCTs are needed to compare postoperative outcomes between SILC and LC.

“…While feasibility has been well demonstrated, there is still a lack of data comparing the new technique with the gold standard of multiport laparoscopic cholecystectomy. Several retrospective studies have been published so far [7][8][9][10][11][12][13][14]. They report no difference in complication rates and judge SILC as a safe procedure resulting in a comparable length of hospitalization as in LC.…”

“…A recently published systematic review of the literature identified 29 studies which included a total of 1,166 SILC (single incision laparoscopic cholecystectomy) procedures with satisfactory results [6]. There are some studies comparing SILC with the gold standard of laparoscopic cholecystectomy (LC) [7][8][9][10][11][12][13][14]. The aim of our study was to challenge the gold standard of LC by SILC in a matched pair analysis, testing the hypothesis that using only one incision (SILC) instead of four (LC), and thus reducing operative trauma, might lead to a reduction in postoperative pain, length of stay, and to an improvement in cosmetic outcome as compared to LC.…”

“…Indeed, SILC favors the patients who take cosmetic result into account. However, not many surgeons believe that SILC can replace TMLC as the standard procedure for gallbladder surgery [70]. The main limitation for widespread promotion of SILC is technically challenging, such as limited triangulation and freedom of movements of instruments [71][72][73], and the critical view of the surgical field.…”

“…Various methods have been proposed for effective traction and suture of the gall bladder fundus by using the Keith needle or hook has been attempted. 17,18 In addition, despite the introduction of flexible instruments and camera systems to address this problem, collision of the laparoscopic instruments remains a problem in manipulating tissues or in delicate movements. For this reason, this disadvantage tends to increase the operating time and learning curve.…”
