Paraneoplastic Sarcoidosis in Multiple Myeloma

Abstract: Sarcoidosis predominantly affects the lungs, intrathoracic lymph nodes, and eyes; it less frequently affects the musculoskeletal system. We herein report a case of paraneoplastic sarcoidosis in a patient presenting with multiple myeloma. The patient developed ocular sarcoidosis and showed an increased 18F-fluorodeoxyglucose uptake in the mediastinal lymph nodes and vertebral column. A lymph node specimen showed the histological features of sarcoidosis, while an examination of the vertebral tumor revealed myelo… Show more

“…In our review of the literature, 33 cases with both sarcoidosis and MM were identified although patient characteristics and diagnostic information were not available for all of them [4, 625]. Data analyses showed female gender predominance, with a median age at sarcoidosis diagnosis of 46 years (24–65).…”

“…The association between sarcoidosis and lymphoma is well established and the sarcoidosis-lymphoma syndrome was first described in 1986 by Brincker and Wilbek 8. However, there are very few reports of an association between sarcoidosis and multiple myeloma 9–12. Herein, we report a case of paraneoplastic sarcoidosis occurring in a patient progressing from smouldering to symptomatic multiple myeloma.…”

“…Additionally, there exists the entity of “sarcoid reactions,” which are localized forms of noncaseating granulomas of adjacent lymph nodes. There appears to be a reaction to antigens draining from the malignancy rather than an actual systemic form of sarcoidosis 5, 6…”
