Atraumatic Subdural Hematoma in a Third-Trimester Gravid Patient

Abstract: Acute atraumatic subdural hematoma is a rare occurrence and there exist few case studies which describe suspected cases and causes for this condition. We present a case of a 36-year-old female at 32-week gestation who initially presented to the emergency department for evaluation of lower extremity cellulitis but had acute neurologic change while being in the ED. Computed tomography revealed a right subdural hematoma with midline shift and mass effect. The primary cause for the patient's subdural hematoma is u… Show more

“…[10] HELLP syndrome happens ASDH: Acute subdural hematoma, GCS: Glascow Coma Scale, HELLP: Hemolysis, elevated liver enzymes, and low-platelet, NM: Not mentioned in pregnant and postpartum individuals characterized by hemolysis with microangiopathic features, elevated liver enzymes, and a low platelet count. [7,17,19] In the acute setting, ASDH management includes either urgent surgical evacuation or conservative management. ASDH more than 10 mm thickness or midline shift more than 5 mm on computed tomography imaging, neurological deterioration, and/or increased intracranial pressure are indications for surgical intervention.…”

“…Em raros casos ocorrem este tipo de hemorragia de forma espontânea, sem evidência de trauma 5,6 . Existem descrições na literatura como causa de hematoma subdural agudo sem trauma prévio, como por exemplo: a rotura de aneurisma, o hematoma intracerebral que invade o parênquima cerebral e ultrapassa a membrana aracnóide, e o embolismo séptico e asséptico ocasionando o sangramento 5 .…”

“…Previously described causes of spontaneous prenatal SDH include ruptured aneurysms, preeclampsia, arteriovenous malformations, cocaine use, and coagulopathy, with pregnancy‐associated hypertension and preeclampsia appearing to be the most common contributing factors. 1 , 2 , 3 In this case report, the authors report on a patient with non‐traumatic SDH of unknown origin in the third trimester of pregnancy.…”
