BACKGROUND AND OBJECTIVES:Identify the level of knowledge about the neurophysiology of pain in patients with chronic migraine through the application of the Neurophysiology of Pain Questionnaire. METHODS: A cross-sectional study was carried out with 30 patients with chronic migraine. The level of neurophysiological knowledge of pain was assessed by the Brazilian version of the Neurophysiology of Pain Questionnaire. RESULTS: In general, patients with chronic migraine had a low level of knowledge about pain neurophysiology with 4.03±1.52 correct answers in 12 questions (34.1%). From those 30 patients, there was a predominance of females (72.7%), aged 33±8.60 years, higher schooling level (73.4%), mild intensity of pain at rest (2.83±2.39), predominant subclassification of chronic migraine without aura (56.7%) and with a history of migraine for more than 10 years (60.1%). This article identified the level of knowledge about neurophysiology of pain in chronic migraines through the application of the Neurophysiology of Pain Questionnaire. CONCLUSION: Patients with chronic migraine have a low level of neurophysiological knowledge.

Introdução: Estudos indicam que acadêmicos de vários semestres da fisioterapia apresentam limitações no conhecimento relacionado à dor e suas terapêuticas. Contudo, essa temática ainda é pouco explorada em acadêmicos no último ano da graduação em fisioterapia. Objetivo: Descrever o conhecimento sobre dor por parte dos acadêmicos do último ano de uma instituição privada de ensino superior. Métodos: Estudo descritivo transversal com 59 acadêmicos de ambos os sexos, matriculados no estágio curricular supervisionado, do último ano do curso de fisioterapia de uma instituição privada de ensino superior. Todos os participantes foram submetidos a um questionário validado contendo questões relacionadas à fisiopatologia, subjetividade, avaliação e terapêuticas não farmacológicas da dor. Resultados: Cinquenta e seis voluntários compuseram a amostra final deste estudo. 88% deles indicaram não existir uma disciplina ou um professor especialista sobre dor. 58% dos acadêmicos não reconhecem a dor como um sinal vital. Houve conhecimento satisfatório sobre conceitos básicos relacionados à fisiopatologia, avaliação e tratamento da dor. Contudo, falhas que comprometem o manejo da dor foram detectadas. Conclusão: Existe inconsistência no conhecimento de acadêmicos do último ano do curso de fisioterapia sobre aspectos ligados a avaliação e tratamento de pacientes com dor.Palavras-chave: conhecimento, dor crônica, Fisioterapia, medicina física e reabilitação.

“…In general, healthcare professionals have poor knowledge about pain neuroscience mechanisms ( 12 ) and reliable ways of assessing it ( 13 , 14 ). This poor knowledge goes against the International Association Study of Pain (IASP) recommendation in the declaration of the Montreal meeting, which highlights that all people with pain have the right to have access to appropriate assessment and treatment of the pain by adequately trained healthcare professionals ( 15 ). Thus, considering the potential relation between CJP and central maladaptive plasticity in SCD individuals and the deficit in healthcare professionals' knowledge about pain neuroscience mechanisms and pain assessment, this review aims to highlight the mechanisms of CNS maladaptive plasticity that might be related to CJP in SCD and propose a battery for reliable musculoskeletal assessment adapted to those patients.…”

Chronic joint pain (CJP) is among the significant musculoskeletal comorbidities in sickle cell disease (SCD) individuals. However, many healthcare professionals have difficulties in understanding and evaluating it. In addition, most musculoskeletal evaluation procedures do not consider central nervous system (CNS) plasticity associated with CJP, which is frequently maladaptive. This review study highlights the potential mechanisms of CNS maladaptive plasticity related to CJP in SCD and proposes reliable instruments and methods for musculoskeletal assessment adapted to those patients. A review was carried out in the PubMed and SciELO databases, searching for information that could help in the understanding of the mechanisms of CNS maladaptive plasticity related to pain in SCD and that presented assessment instruments/methods that could be used in the clinical setting by healthcare professionals who manage chronic pain in SCD individuals. Some maladaptive CNS plasticity mechanisms seem important in CJP, including the impairment of pain endogenous control systems, central sensitization, motor cortex reorganization, motor control modification, and arthrogenic muscle inhibition. Understanding the link between maladaptive CNS plasticity and CJP mechanisms and its assessment through accurate instruments and methods may help healthcare professionals to increase the quality of treatment offered to SCD patients.
