Objective: Dry eye is one of the complications of hospitalization in ICUs. This study was conducted to assess the effectiveness of a Balanced Salt Solution (BSS) on dry eyes in comatose patients. This clinical trial was conducted in 2019 on 34 patients admitted to the ICUs. The control group received sterile eye ointment with artificial tears (2 drops every 6 hours) and a tape over the eye. The trial group also received BSS (2 drops every 6 hours) in the opposite eye over five days. The patients' dry eye was examined on first day and sixth day of the study using Schirmer’s Test (ST) and the Tear Breakup Time Test (TBUT). The data were analyzed in SPSS-21.Results: The results of the paired T-test based on ST and TBUT showed no significant change from first day one to sixth day in the BSS-receiving group (P>0.999 and P=0.187, respectively). Furthermore, no significant difference was observed between the two groups in the mean score of dry eye after eliminating the effect of the demographic and clinical variables (P=0.947). The administration of this solution by nurses seems to be an effective method for preventing the progression of dry eye in ICU patients.

Dry eye disease (DED) is a multifactorial ocular condition characterized by a loss of homeostasis of the tear film resulting in ocular symptoms of discomfort, irritation, and visual disturbance, all of which significantly impact the patients' social and occupational quality of life. While management of DED depends on the severity of symptoms and signs, use of artificial tear products (ATPs) that replace or supplement the deficient natural tear film is the mainstay treatment option. In this review, we present a decade of evidence on Systane Ultra ® (polyethylene glycol [PEG]/propylene glycol [PG] with hydroxypropyl guar [HP guar]) in effectively managing DED. The active demulcents in Systane Ultra ® -PEG, PG, along with HP guar gelling technology-provide optimal ocular surface protection and lubrication to heal damaged areas of the cornea caused by DED and, therefore, are recommended for patients with both aqueous and/or mucin layer deficiencies. Over the years, several clinical studies have shown that PEG/PG with HP guar provides long-lasting relief from dry eye and has often been chosen as a standard or comparator against other ATPs. Here, we describe the salient features of PEG/PG with HP guar-its constituents and their mechanisms of action. Furthermore, we summarize results from a systematic literature search that identified 23 relevant publications further emphasizing on the effectiveness and safety of PEG/PG with HP guar in alleviating the signs and symptoms of DED.

The purpose of this article is to provide a framework for general ophthalmologists in Singapore to manage dry eye. This framework considers the evidence in the literature as well as recommendations from expert panels such as the Tear Film & Ocular Surface Society Dry Eye Workshop II and the Asia Cornea Society Workgroup. This article covers the assessment of patient medical history and ask triage questions to identify local and systemic causes of dry eye disease (DED), excluding other possible causes, as well as the risk factors for DED and ocular surface inflammation. Evaluation of clinical signs to establish the diagnosis of DED and differentiation from other causes of irritable, red eyes are described. Tests for understanding the underlying disease processes and severity of DED are also presented. Management of dry eye should involve patient education and engagement. Information about the natural history and chronic nature of DED should be provided to improve long-term management of the disease and enhance compliance. Aggravating factors should be removed or lessened. We provide a guide to determine the most appropriate treatment (or combination of treatments) based on the severity and cause(s) of the disease, as well as the patient's needs and preferences. The aim of the management is to relieve ocular discomfort and prevent worsening of symptoms and signs, as well as to optimize visual function and minimize structural ocular damage. We also discuss the systematic follow-up and assessment of treatment response, as well as monitoring side effects of treatment, bearing in mind continuous support and reassurance to patients.
