O objetivo deste estudo foi analisar os determinantes do envelhecimento saudável em um grupo de idosos assistidos pela Estratégia Saúde da Família de Belém/Pará. Estudo transversal, realizado com 279 idosos cadastrados em uma Estratégia Saúde da Família na cidade de Belém-PA, no período de janeiro a março de 2014. Os dados foram coletados segundo os determinantes do envelhecimento saudável (transversais e econômicos, sociais, ambientais e da saúde). Para a análise dos dados utilizou-se o Programa Estatístico Statistical Package for Social Science versão 20.0. Dentre os resultados obtidos têm-se que entre os idosos, 65,2% eram do sexo feminino e 34,8%, do sexo masculino. A maior proporção deles estava na faixa etária de 60 a 69 anos; 53,0% tinham baixa renda e 59,5%, baixa escolaridade. Como fatores de risco ao envelhecimento identificaram-se quedas em 52,0%, mais de 75% apresentavam algum agravo à saúde; 70,3% das mulheres eram sedentárias e cerca de 63,7% delas se hospitalizaram. Evidenciou-se neste estudo que a maioria dos idosos apresentou prejuízo para o envelhecimento saudável, como risco de quedas, sedentarismo e baixa renda, destacadamente entre as mulheres. Nesse sentido, a contribuição do manuscrito para a saúde coletiva consiste na apresentação do cenário que se desenha o envelhecimento saudável no âmbito de uma ESF, permitindo o planejamento de intervenções específicas.

Sociodemographic profile of primiparous mothers from nine birth cohorts in three Brazilian cities

Fertility reduction is a phenomenon observed in demographic transition. The demographic changes noted in female fertility represent a need for adjustment on health services regarding female health and family planning support. Thus, this study aimed to perform a descriptive analysis by tracing the sociodemographic profile of primiparous mothers belonging to nine Brazilian birth cohorts, in three cities from different states. Standardized questionnaires were applied to assess reproductive characteristics and covariables. Primiparous mothers were defined as women whose child included in birth cohorts was their firstborn child. Sample description was performed using analysis of variance (continuous variables) and chi-square (categorical variables). In total, 44,615 women were included in the analyses and 41.8% (95%CI: 41.3; 42.2) were categorized as primiparous. The primiparity rates were the lowest in Ribeirão Preto (São Paulo State) 1978 (32%; 95%CI: 30.9; 33.1) and the highest in most recent cohorts, reaching up to 50% of the participants (São Luís - Maranhão State 2010: 47.2%; 95%CI: 45.8; 48.6; Ribeirão Preto 2010: 50.2%; 95%CI: 49.1; 51.4); Pelotas (Rio Grande do Sul State) 2015: 49.4% (95%CI: 47.9; 50.9). Primiparous mothers’ age and schooling increased over the years in all cohorts. Maternal age at the first childbirth behaved similarly in the three studied cities. There was an increase in the proportion of first-time mothers that were older, higher educated and belonged to richer income groups. Also, the proportion of teenage mothers (aged 15 years or younger) increased until the early 2000’s and started decreasing around the years 2010, especially among women in the poorer income groups.

The objective of this study was to investigate intergenerational breastfeeding practices according to parental sex and age at delivery in the 1993 Pelotas (Brazil) birth cohort study. This is a prospective birth cohort study, and at the 22‐year follow‐up, a substudy with all children of the cohort members who had become parents was conducted (93Cohort‐II). First generation breastfeeding data were collected at 3 months and 4‐year‐old follow‐ups. In the 93Cohort‐II, parents answered a questionnaire about their children's breastfeeding practices. Adjusted Tobit and Poisson regression models with robust variance were applied to estimate the association between predominant parental breastfeeding duration and exclusive breastfeeding duration of the children at 3 and 6 months. Out of 3,810 cohort participants, 955 (25%) had delivered at least one live‐born infant, and 1,222 children were assessed. Fifty‐four percent of parents were ≤19 years old. Direct effects of predominant parental breastfeeding duration on exclusive breastfeeding duration of their children were only observed when data were stratified by parental age: children born to parents aged ≥20 years old and who were predominantly breastfed for at least 3 months presented higher exclusive breastfeeding duration and higher prevalence of being exclusively breastfed for at least 3 months. When analyses were stratified by mothers and fathers, the result remained significant only among mothers. Longer predominant breastfeeding duration in the first generation was associated with longer exclusive breastfeeding duration in the second generation, but only among older mothers. Education and social support surrounding breastfeeding should be intensified among fathers and younger parents to create a positive environment supportive of breastfeeding.
