Objetivo: Verificar a relação entre variações do clearance de creatinina (ClCr) e os fatores clínicos de pacientes internados em clínica médica. Material e Método: Estudo longitudinal, do tipo coorte prospectivo, quantitativo, realizado em um hospital público do Distrito Federal. 85 pacientes evoluíram com Lesão Renal Aguda (LRA), de acordo com a classificação Kidney Disease: Improving Glogal Guidelines (KDIGO). O acompanhamento foi até 1 mês pós-alta hospitalar. A gravidade dos pacientes foi medida através do Índice de Comorbidade de Charlson. Considerou-se significativo resultados com p ≤ 0,05. Resultados: 51,8% dos pacientes eram do sexo masculino. Entre as comorbidades se destacaram hipertensão arterial (70,6%), diabetes mellitus (57,6%) e as cardiopatias (52,9%). O tempo de permanência hospitalar (p= 0,001) e a idade (p= 0,05) estiveram associadas a pior função renal. Pacientes com ClCr≤ 30 ml/min estiveram associados a uma maior taxa de mortalidade (p= 0,007). Conclusão: Idade avançada e maior tempo de internação hospitalar se associaram a piora da função renal (ClCr < 60 ml/min). Àqueles com severa piora da função renal (ClCr < 30 ml/min) apresentaram maior taxa mortalidade.

Influence of variation of the serum creatinine on outcomes of patient with acute kidney injury

Objective: to identify the influence of variations on serum creatinine on outcomes of patients with acute kidney failure. Methods: observational and prospective study. Eighty-five patients which evolved with renal impairment composed the sample. The data was extracted from medical records and reported in a questionnaire created previously. It was considered values p≤0.05 as significant. Results: most of patients were male gender (51.8%) and elder (66±14 years). The reduction of kidney function occurred in different graduation, considering that 35.3% evolved with risk to kidney injury (stage 1) and lower percentage (14.1%) was more severely affected (stage 2 and 3), respectively. During follow-up, the clearance creatinine progressively increased showing recovery of kidney function. Patients with renal injury or failure (stage 2 or 3) evolved more frequently to death (p=0,027). Conclusion: variations on serum creatinine may exacerbate the patient outcomes over hospitalization period. Descriptors: Acute Kidney Injury; Creatinine; Mortality. Objetivo: identificar a influência de variações na creatinina sérica sobre o desfecho dos pacientes com lesão renal aguda. Métodos: estudo observacional prospectivo. A amostra foi composta de 85 pacientes que evoluíram com comprometimento renal. Os dados foram extraídos do prontuário e registrados em questionário construído previamente. Valores com p≤0,05 foram considerados significativos. Resultados: a maioria dos pacientes era do sexo masculino (51,8%) e idosos (66±14 anos). A redução da função renal ocorreu em graduações diferentes, haja vista que 35,3% evoluiu com risco para lesão renal (estágio 1) e um menor percentual (14,1%) foi acometido mais gravemente (estágios 2 e 3), respectivamente. O clearance de creatinina aumentou progressivamente revelando recuperação da função renal ao longo do acompanhamento. Pacientes com lesão ou falência renal (estágios 2 ou 3) evoluíram mais frequentemente ao óbito (p=0,027). Conclusão: variações na creatinina sérica podem agravar o desfecho do paciente ao longo do período de internação. Descritores: Lesão Renal Aguda; Creatinina; Mortalidade.

Gout, typically manifesting as acute burning pain and swelling in a joint, has a high frequency of comorbidities. Based on Traditional Chinese Medicine syndrome (TCMS) theory, obstruction of dampness and heat syndrome (ODHS) and intermingled phlegm-stasis blood syndrome (IPSBS) were the two main TCMS subtypes in Chinese suffering from acute gout. In this study, we did a retrospective study enrolling 4,417 ODHS male gout cases and 1,413 IPSBS male gout cases, to investigate the comorbidities distribution difference between the two subtype groups and seek the potential indicators of male gout with some comorbidities. Interestingly, we found male ODHS group with higher prevalence of possible kidney damage (ODHS: 4.34%; IPSBS: 0.78%), lower prevalence of cardiac-cerebral vascular diseases (ODHS: 0.52%, IPSBS: 0.85%) and diabetes (ODHS: 1.06%; IPSBS: 1.63%) than male IPSBS group. And cystatin C is the only index reflecting that renal function showed significant difference between the two groups and the average levels were out of the normal range (1.09 ± 0.28 versus 1.17 ± 0.31, p=0.001). Further, we also observed significance difference on abnormality rates of cystatin C between the two groups. (χ2=5.543, p= 0.019). Besides, the comparison between the two subtypes also showed significant difference on hematocrit (43.12 ± 3.60 versus 42.26 ± 4.17%, p=0.007), mean corpuscular volume (89.52 ± 6.07 versus 86.81 ± 7.11fL, p=0.001), and mean corpuscular hemoglobin concentration (338.00 ± 11.67 versus 334.86 ± 13.58g/L, p=0.004). In general, we put forward that male gout patients with ODHS should be more vigilant of damage of renal function, and those with IPSBS should pay more attention to prevent cardiac-cerebral vascular diseases and diabetes. Increased Cys C level might be correlated with risk of comorbidities, especially diabetes . Thus, it is of significance to diagnose the TCMS in acute gout accurately and monitored related indices to prevent comorbidities.
