Shiitake dermatitis: the first case reported in Brazil

Abstract: Shiitake Dermatitis is often presented as papules and erythemato-violaceous linear streaks. It can be associated with bleomycin treatment, dermatomyositis and shiitake mushroom (Lentinus edodes). There is not any previous report concerning this rare etiology in our country. Shiitake is the second most consumed mushroom worldwide and it can cause flagellate erythema when ingested raw or half cooked. It has a higher incidence in Oriental countries because of their eating habits, this is the first case reported i… Show more

“…Conversely, the fact that this rash usually occurs after the ingestion of large amounts of raw mushroom and not after limited exposures suggests that there may be a dose‐dependent effect, which is consistent with a toxic mechanism . However, we favor a hypersensitivity reaction and postulate that genetically susceptible individuals with certain human leukocyte antigen (HLA) alleles require exposure to large amounts of lentinan in order to be sensitized.…”

“…Flagellate erythema, a peculiar cutaneous adverse effect of bleomycin or its derivative peplomycin, occurs in 8-20% patients during systemic or local therapy (1). It characterised by a linear erythematous streak with hyperpigmentation (2, 3), and was first described by Moulin in 1970 (4). Herein, we present a typical case of flagellate erythema induced by Bleomycin.…”
