Objectives: to identify the prevalence and determining factors of the complements in offering food to newborns. Methods: across-sectional study nested to a cohort study that assessed newborn care in four public maternity hospitals in Natal/Brazil. Sample was composed by 415 mothers and full-term newborns, with appropriate weight for gestational age and Apgar scores in 1st and 5th minutes ≥ 7. In order to analyze the determining factors, we used Poisson’s regression. Results: from 415 newborns, 51.3% received feeding complements (57.6% in the first hour of life), of which 92% was infant formula. Only 50.7% of those complement in offering food were prescribed by physicians. Colostrum deficiency was the main reason to be indicated (33.8%). Maternal age ≤ 20 years old (PR=0.64; CI95%=0.47-0.86) and between 2030 years old (PR=0.70; CI95%=0.57-0.87)comparing to women older than 30 were shown as protective factors, while being primiparous (PR=1.37; CI95%=1.11-1.60) and had cesarean section (PR=1.2; CI95%=1.00-1.45) as risk factors. Conclusion: maternal characteristics are associated with the complement in offering food to the newborn in the first hours of life. The high prevalence shows the need for interventions that minimize the inadequate offer of infant formula and promote exclusive breastfeeding before hospital discharge.

Objetivo: avaliar o desempenho de nutrizes e recém-nascidos, durante a mamada, no período neonatal e identificar as dificuldades para a prática do aleitamento materno/amamentação. Método: estudo longitudinal, comparativo, realizado em hospital regional, no Piauí, Brasil. Participaram 49 nutrizes, abordadas no primeiro e 28° dia de vida dos recém-nascidos. A coleta aconteceu entre setembro de 2018 e fevereiro de 2019, com questionário de caracterização e aplicação do instrumento LATCH – Scoring System (Latch, Audible swallowing, Type of nipple, Comfort, Hold). Para análise, utilizou-se estatística descritiva e testes de associações múltiplas. Resultados: verificou-se que, na primeira avaliação, 32,7% das nutrizes e recém-nascidos necessitaram de suporte para adequação do processo de amamentação, enquanto na segunda, apenas 2%. Conclusão: evidenciou-se que intervenções precoces voltadas para promoção e manutenção do aleitamento materno devem ser implementadas por enfermeiros, com início no pré-natal e indispensáveis na primeira semana puerperal, especialmente relacionadas à técnica de amamentação.

Summary Context There is an increasing prevalence of overweight during early childhood in the most impoverished areas in Brazil, although there is a lack of evidence regarding its onset. Objectives To investigate the incidence and risk factors associated with overweight among low‐income children during their first year of life. Methods We analysed data from a prospective birth cohort study conducted in Brazil, which followed‐up children at birth, 3rd, 6th and 12th months (n = 196). The overweight incidence (zBMI/A > 2SD) was analysed using the Kaplan–Meier survival estimator and its associated factors by the Coxs regression model. Missing data were addressed with multiple imputations and results on the final adjusted model were calculated by pooling the estimates generated for each imputed dataset. Results The overweight incidence was 17 events/100 children‐year, and most cases occurred before the 6th month. The final model showed that children exclusively breastfed up to 30 days (vs. >30 days; HR 2.68; 95%CI 1.11–6.49) and whose mothers consumed ultra‐processed foods more than 4 times/day (vs. ≤4 times/day; HR 3.02; 95%CI 1.28–7.13) presented a higher risk of developing overweight. Conclusion The overweight incidence was high in this impoverished population. Shorter exclusive breastfeeding duration and an unhealthy household food environment provided a short‐term risk increase.
