bCystic fibrosis (CF) is the most frequent lethal genetic disease in the Caucasian population. Lung destruction is the principal cause of death by chronic Pseudomonas aeruginosa colonization. There is a high prevalence of oropharyngeal anaerobic bacteria in sputum of CF patients. This study was carried out due to the lack of results comparing subgingival periodontal pathogenic bacteria between the oral cavity and lungs in patients with CF in relation with P. aeruginosa presence. Our first goal was to detect P. aeruginosa in oral and sputum samples by culture and molecular methods and to determine clonality of isolates. In addition, subgingival periodontal anaerobic bacteria were searched for in sputum. A cross-sectional pilot case-control study was conducted in the CF Reference Center in Roscoff, France. Ten CF patients with a ⌬F508 homozygous mutation (5 chronically colonized [CC] and 5 not colonized [NC]) were enrolled. P. aeruginosa was detected in saliva, sputum, and subgingival plaque samples by real-time quantitative PCR (qPCR). Subsequently, periodontal bacteria were also detected and quantified in subgingival plaque and sputum samples by qPCR. In CC patients, P. aeruginosa was recovered in saliva and subgingival plaque samples. Sixteen P. aeruginosa strains were isolated in saliva and sputum from this group and compared by pulsed-field gel electrophoresis (PFGE). Subgingival periodontal anaerobic bacteria were found in sputum samples. A lower diversity of these species was recovered in the CC patients than in the NC patients. The presence of the same P. aeruginosa clonal types in saliva and sputum samples underlines that the oral cavity is a possible reservoir for lung infection.

Hospitalized patients and patients treated in Intensive Care Units (ICUs) are at high risk of contracting infectious diseases, mainly lung diseases resulting from respiratory pathogens found in the oral cavity due to deficient oral health maintenance. This problem could be avoided through preventive actions that require minimum intervention. The surface of the tooth, tongue and prostheses, as well as the ICU devices in contact with the patients' mouth, is large reservoirs for pathogens. Thus, cleaning the patients' mouth and removing infection foci may positively influence the quality of life and well-being of critically ill patients. The aim of the current study is to address the importance of having dentists in the health teams working in ICUs, as well as the activities performed, the necessary dental care and the relation between oral and systemic conditions. The study was approved by the Ethics Committee of the Catholic University of Brasília, Brazil, under protocol number CAAE 44578215.0.0000.0029. It was concluded that the lack of preventive measures, as well as the deficient oral cavity hygiene and clinical care in ICUs, are risk factors for the development of systemic diseases, mainly of diseases affecting the respiratory and cardiological tracts; therefore, the participation of dental surgeons in the interdisciplinary team is crucial to promote health in hospital environments.

Purpose of Review The understanding of the human microbiome, especially of the oral cavity, has expanded exponentially since the advent of 16 rRNA PCR gene sequencing. Since the respiratory tract starts from the oral cavity and ends in the lung, study of the relationship between the oral microbiota and the lungs will allow us to understand the changes in lung disease compared with healthy state. Recent Findings The oral and lung microbiota were found to be similar, but the oral microbiota had greater diversity. The oral cavity especially the dorsal tongue was found to be a reservoir for bacteria causing pneumonia and chronic lung infection in cystic fibrosis. Oral and lung infections seem to all share the similar pathogenesis of oral microbiota dysbiosis. Certain oral bacteria were found to be potential biomarkers for lung cancer. Summary Improvement in oral health is important especially in the management of lung diseases with infectious etiology. Oral microbiota can serve as biomarkers for diseases especially in lung cancer.
