Introduction: Lower urinary tract dysfunction (LUTD) is a common clinical condition. Emotional and behavioral issues are increasing among children and adolescents, with stress indicating diffi culties in personal and social functioning. This study evaluated whether urinary tract symptoms (LUTS) is associated with stress. Materials and Methods: A cross-sectional, analytical study with 6-14-year-old patients with LUTS and no anatomical/neurogenic urinary tract abnormalities was conducted using the Dysfunctional Voiding Scoring System, a psychological assessment and the Child Stress Scale. The overall stress score was analyzed in relation to the psychological assessment data. Answers to the seven specifi c DVSS urinary questions were compared with those for the four Child Stress Scale domains. Univariate and multivariate analyses were performed. The chi-square test and Pearson's correlation were used to determine associations. Signifi cance was defi ned as p <0.05. Results: Most children were male (56%). Mean age was 9.0±2.25 years. Stress was detected in 20 out of 98 patients (20.4%; 95% CI: 13-30%).Of these, 90% were born from unplanned pregnancies and 67% were upset about their disorder. All the Child Stress Scale domains were signifi cantly associated with urinary dysfunction, with dysuria being signifi cantly associated with all four domains. In the multivariate analysis, dysuria was the only symptom that remained associated with stress. Associations with stress strengthened as the frequency of dysuria increased: physical reactions (p <0.01), emotional reactions (p <0.05), psychological reactions with a depressive component (p <0.01) and psychophysiological reactions (p <0.05). Conclusion: Stress levels are higher in children and adolescents with LUTS who have more severe symptoms. Dysuria was the symptom most associated with stress, both in the physical reactions domain, in the psychological reactions domains with or without a depressive component and in the psychophysiological reactions domain.

As disfunções do trato urinário inferior impactam a qualidade de vida de crianças ao causarem incontinência urinária e infecções do trato urinário. Dentre estas tem destaque a disfunção miccional, caracterizada por um padrão miccional incomum, onde ocorre a perda da capacidade de armazenamento, estocagem e esvaziamento. A fisioterapia emerge como técnica promissora no tratamento destas crianças, mas não há clareza sobre quais técnicas são as mais usadas e mais eficazes neste tipo de tratamento. Revisão sistemática sobre as técnicas fisioterápicas para o tratamento da disfunção miccional infantil nas bases SciELO, LILACS e PubMed, restrita a ensaios clínicos randomizados. De um total de 99 artigos retornados apenas oito tratavam sobre disfunção miccional. As técnicas mais utilizadas foram a uroterapia comportamental (hidratação, diário miccional, treino miccional, dieta) e reeducação do assoalho pélvico, e as mais eficazes foram a uroterapia comportamental junto a redução do assoalho pélvico associada ao biofeedback. A literatura é parca sobre o tema, mas todas as técnicas apresentaram resultados, apesar da grande heterogeneidade metodológica que impediu qualquer análise comparativa mais precisa. A uroterapia comportamental associada ao treinamento dos músculos do assoalho pélvico auxiliado por biofeedback foi a técnica que apresentou melhores resultados.

Objective: Uroflowmetry is an essential noninvasive test with important diagnostic method in patients with initial diagnosis of lower urinary tract dysfunction. We aimed to compare the results of conventional uroflowmetry with those of the new smart "self-directed outpatient" uroflowmetry in children with suspected lower urinary tract dysfunction. Methods: This cross-sectional study included children who had performed two sequential urinations in the same day recorded by conventional and smart "self-directed outpatient" uroflowmetry. Results of the measurements of maximum, and average urinary flow rates were recorded and compared. Results: The mean difference between average urinary flow rates detected by both diagnostic methods was-1.7. The Bland-Altman plot showed that most of the data points were tightly clustered around the zero line of the difference between the measurements, with only 4% of the readings falling outside the 95% level of confidence. The mean difference between average urinary flow rates measured by both conventional and "self-directed outpatient" uroflowmetry was-4.5. Discussion: The maximum, and average urinary flow rates measured with "self-directed outpatient" uroflowmetry were statistically significantly higher compared to conventional uroflowmetry. These results could be due to the children being much shyer and being affected by the presence of someone in the room while urinating. Conclusion: The maximum urinary flow rate and average urinary flow rate measured with "self-directed outpatient" uroflowmetry are higher compared to conventional uroflowmetry, which might ensure patient privacy.
