BACKGROUND: No standards reflecting the quality of life (QOL) and activity of daily living (ADL) in postoperative clinical course have been validated in the area of vascular disease. The Walking Impairment Questionnaire (WIQ) is a disease-specific questionnaire that evaluates patients with intermittent claudication due to arteriosclerosis obliterans (ASO). The WIQ uses four subscales: pain severity, walking distance, walking speed, and stair climbing while walking. OBJECTIVE: To evaluate the correlation between postoperative arterial blood flow and the Japanese edition of the WIQ. METHODS: Thirty-one patients (47 limbs) with intermittent claudication who had been subjected to lower limb surgical arterial reconstruction were assessed by WIQ, and compared with Ankle-Brachial Pressure Index (ABPI) and angiography. RESULTS: A significant increase in the WIQ score was identified in walking pain (26 versus 89.5, p<0.001), walking distance (13.1 versus 83.3, p<0.001), walking speed (10 versus 46, p<0.001), and stair climbing (6.2 versus 79, p<0.001). The correlation coefficient between the increase in postoperative ABPI and the WIQ score was R²=0.1889, which shows weak correlation. The correlation between blood flow obstruction due to the postoperative bypass that was scored by angiography and WIQ score was R²=0.3894, which shows moderate correlation. CONCLUSION: An improvement in the Japanese edition of the WIQ score was correlated not only with the patients' QOL after bypass revascularization but also with the rate of increase of postoperative ABPI and image findings, such as the postoperative angiography.

Background:The time at which claudication symptoms are reported is used to modulate exercise intensity in clinical treatment of patients with peripheral arterial disease, but tissue oxygenation values at that point are unknown. Objective: To describe tissue oxygen supply measured using Near-Infrared Spectroscopy (NIRS) when patients report initial and maximum claudication symptoms during exercise tests. Methods: Nine patients (eight men) aged 65.63 ± 6.02 years and previously diagnosed with peripheral arterial disease performed constant load exercise testing and incremental load exercise testing while tissue oxygenation levels were monitored by NIRS. Oxygen saturation values at the times at which each patient reported initial onset of claudication symptoms and maximum claudication symptoms were compared with values obtained during the arterial occlusion maneuver, using the 95% confidence interval of the difference. Results: It was found that saturation values at the time of both initial and maximum claudication symptoms were statistically different from saturation during the arterial occlusion maneuver, but on the basis of percentage analysis they were similar from a clinical point of view. Conclusions: Oxygen saturations at the time patients report initial and maximum claudication symptoms are very close to saturations during arterial occlusion. From a clinical perspective, subjective patient report of symptoms is an appropriate parameter on which to base exercise prescription.

CONTEXTO: As informações sobre o grau de correlação entre força muscular e capacidade funcional em pacientes com doença arterial obstrutiva periférica (DAOP) são inconsistentes, além de nem sempre apresentarem protocolos que relacionem apropriadamente medidas de força muscular com desempenho. OBJETIVO: Estabelecer o nível de correlação entre força muscular e capacidade funcional em pacientes fisicamente ativos com DAOP. MÉTODOS: Doze pacientes com diagnóstico da doença e presença de claudicação intermitente participaram do estudo. Todos realizaram testes de força muscular e de caminhada. Grau de correlação entre força muscular e capacidade funcional foi avaliado pela correlação de Pearson. RESULTADOS: Dos 12 voluntários incluídos, 10 eram do sexo masculino e 2 do sexo feminino. Os participantes tinham média de idade de 63 ± 11 anos. Houve alta correlação (r = 0,872; p = 0,0001) entre distância percorrida no teste de deslocamento bidirecional progressivo (TDBP) e carga alcançada no teste de resistência máxima. Não se observou correlação entre distância percorrida no TDBP e tempo gasto para realização das cinco flexões plantares no teste de ponta de pé. CONCLUSÃO: O desempenho funcional em um grupo de pacientes com DAOP, em sua maioria fisicamente ativos, foi fortemente correlacionado com força muscular de extensores de joelho, mas não com desempenho observado funcionalmente pelo teste de ponta de pé. Futuros estudos são necessários para avaliar se força muscular de flexores plantares, mensurada de forma específica e isolada, correlaciona-se com função em pacientes com a doença.
