Both the 6MW and the SWT are reliable walking tests for claudicant patients. However, the reliability coefficients obtained with the SWT are higher, the measurements are less variable, and there were better associations between this walking test and hemodynamic variables.
CONTEXTO: As informações sobre o grau de correlação entre força muscular e capacidade funcional em pacientes com doença arterial obstrutiva periférica (DAOP) são inconsistentes, além de nem sempre apresentarem protocolos que relacionem apropriadamente medidas de força muscular com desempenho. OBJETIVO: Estabelecer o nível de correlação entre força muscular e capacidade funcional em pacientes fisicamente ativos com DAOP. MÉTODOS: Doze pacientes com diagnóstico da doença e presença de claudicação intermitente participaram do estudo. Todos realizaram testes de força muscular e de caminhada. Grau de correlação entre força muscular e capacidade funcional foi avaliado pela correlação de Pearson. RESULTADOS: Dos 12 voluntários incluídos, 10 eram do sexo masculino e 2 do sexo feminino. Os participantes tinham média de idade de 63 ± 11 anos. Houve alta correlação (r = 0,872; p = 0,0001) entre distância percorrida no teste de deslocamento bidirecional progressivo (TDBP) e carga alcançada no teste de resistência máxima. Não se observou correlação entre distância percorrida no TDBP e tempo gasto para realização das cinco flexões plantares no teste de ponta de pé. CONCLUSÃO: O desempenho funcional em um grupo de pacientes com DAOP, em sua maioria fisicamente ativos, foi fortemente correlacionado com força muscular de extensores de joelho, mas não com desempenho observado funcionalmente pelo teste de ponta de pé. Futuros estudos são necessários para avaliar se força muscular de flexores plantares, mensurada de forma específica e isolada, correlaciona-se com função em pacientes com a doença.
Low back pain (LBP) is a prevalent condition across cultures, and the tools used for assessment of distinct groups need to be evaluated. Our goal was to investigate the psychometric properties of physical performance tests (PPTs) in a group of Brazilian Portuguese-speaking (BPS) individuals with LBP in a nonexperimental, descriptive study. BPS individuals with and without LBP were studied, and PPTs, self-report questionnaires, and VAS pain score were evaluated. Thirty individuals with LBP (Patient Group (PG)) and 30 healthy subjects (Control Group (CG)) were assessed by 7 PPTs: timed up and go (TUG), loaded forward reach (LFR), 50-foot walk (50 FW), 5-minute walking (5 MW), sit-stand (SS), repeated trunk flexion (RTF), and lumbar flexion (LF). The PG completed the Roland Morris Disability Questionnaire (RMDQ) and the Self-efficacy Scale (SES) and rated their current level of pain by a Visual Analog Scale (VAS). PPTs were highly reliable in the PG with ICC scores ranging from 0.92 to 0.99 (p < 0.0001). The CG performed better in all PPTs but the LFR. In the PG the correlations among the PPT varied from 0.38 to 0.78 (p < 0.05). RMDQ correlated significantly with RTF, SS, 5 MW (0.38 to 0.47, p < 0.05). PPTs were reliable with the BPS patients and able to differentiate groups. RMDQ complements the assessment of these patients.
All but C generated high coefficients of reliability. All variables demonstrated acceptable test-retest variability. There was low variability between participants for C.
CONTEXTO: Uma vez que a obstrução arterial periférica pode se apresentar de maneira difusa, com clínica diversa e com resultados de intervenção variados, é fundamental que a avaliação dos pacientes com doença arterial obstrutiva periférica seja feita com instrumentos que possam apresentar dados objetivos e reprodutíveis. OBJETIVO: Investigar e contrastar a confiabilidade do teste de caminhada de 6 minutos (T6M) com teste de deslocamento bidirecional progressivo (TDBP) em indivíduos claudicantes portadores de doença arterial obstrutiva periférica. MÉTODOS: Quatorze pacientes em estágio II de Fontaine participaram deste estudo piloto. Onze pacientes realizaram ambos os testes e três realizaram apenas T6M. Após familiarização, os pacientes foram avaliados em duas ocasiões distintas com intervalo máximo de 1 semana entre si. O coeficiente de correlação de intraclasse (ICC2,1) foi utilizado para avaliação da reprodutibilidade teste-reteste. RESULTADOS: A média da distância máxima de caminhada no teste e no reteste no T6M foi de 397,04±120,74 e 408,6±153,64 metros (p = 0,58), respectivamente, com ICC = 0,87 (p = 0,00005); já no TDBP, a média foi de 345±145,75 metros e, no reteste, de 345,91±127,97 (p = 0,92), com ICC = 0,99 (p = 0,00005). O tempo médio para surgimento da dor inicial, em segundos, com o T6M, foi de 172,25±88,23 (teste) e 148,58±70,36 (reteste) (p = 0,13), com ICC = 0,81 (p = 0,0004). No TDBP, o tempo médio foi de 282±141,90 (teste) e 267,14±150,58 (reteste) (p = 0,55), com ICC = 0,91 (p = 0,0008). CONCLUSÃO: Ambos os testes de caminhada são confiáveis e úteis para avaliação clínico-funcional desses pacientes. O TDBP, entretanto, gerou índices de confiabilidade mais elevados, podendo ser melhor opção para avaliação da performance desses indivíduos.
