A total of 54 articles published between 2002 and 2012 in 20 different journals were identified. Ten of these were retrieved from the references section of other articles. Focusing mostly on children from the Southeast region, these studies were coordinated by dieticians, nurses, and physicians. Diabetes Mellitus, high blood pressure, and breastfeeding were the most common topics (n=23). The quantitative methodology was employed by 42 articles, most about diagnoses. Only five research groups studied the Family Health Strategy, despite the growing number of studies in the area over the years.

In addition, surveys among samples of adult population in Brazilian capitals have indicated that 60% of adults do not engage in any kind of leisure physical activity and about 85% of people do not practice at least 150 minutes of physical activity in this domain [6][7] . AbstractThe present study aimed at: 1) describing the prevalence of actions to promote physical activity and healthy eating and describing the characteristics of family health strategies in Brazilian municipalities funded to implement the "Health Gym Program"; and 2) verifying the prevalence of these actions according to the presence of Family Health Support Centers (FHSC) and the presence of physical education professionals and nutritionists in the health family teams. We conducted telephone interviews with public health managers in 2012 in all cities funded to implement the "Health Gym Program". We described the frequencies (%) of the family health teams' characteristics and of the physical activity and healthy eating actions, and we calculated the prevalence in municipalities with and without FHSC, and in municipalities with and without physical education professionals and nutritionists in the teams. We used the chi-square test to evaluate the associations. Out of a total of 5,570 municipalities in Brazil in 2012, 2,074 (37.2%) were funded to implement the program, and amongst them, 44.1% (n = 914 public health managers) participated in the survey. Most of the municipalities did not have FHSC teams (61.5%), though reported developing physical activity (84.1%) or healthy eating actions (83.9%). The education sector was referred to as a public partner in most actions. The prevalence of physical activity (91.5%, p < 0.001) and healthy eating (88.2%, p = 0.006) actions was higher in cities with FHSC than in their counterparts. The prevalence of primary care actions was higher in municipalities that had physical education professionals and nutritionists in teams. Having FHSC with physical education professionals and nutritionists is an important factor for promoting physical activity and healthy eating.

Evaluation of nutritional care of overweight adults from the perspective of comprehensive health care

Objective: Describe and evaluate the nutritional care provided for overweight adults by the Primary and Secondary Health Care services of Santos, São Paulo, Brazil. Methods: This study was carried out between 2013 and 2015 integrating quantitative and qualitative approaches; it was divided into two phases: (1) characterization and (2) evaluation. In phase 1, a census of Primary Health Care Units (n=28) and Secondary Health Care Units (n=4) was conducted using interviews with health service managers and/or health professionals. Data were analyzed using exploratory data analysis. In phase 2, in-depth interviews were conducted with health service managers and/or health professionals investigating a sample of the Primary Health Care services and the totality of Secondary Health Care services provided. Thematic analysis was carried out using the theoretical framework for comprehensive health care proposed by Pinheiro & Mattos. Results: A total of 40 professionals were interviewed: 36 in the primary health care services and 4 in the secondary health care services. Nutritional care in the Primary Care services is focused on individual care and referrals to other services; nutrition diagnosis and health promotion occur only when overweight is associated with another disease. It was observed that the referral and counter-referral system and intersectorial collaborations were ineffective. In Secondary Care services, nutritional care is focused on clinical care using traditional approaches to nutrition education. Limiting factors for promoting comprehensive care were identified at the two levels of care: unproductive actions, lack of actions for health promotion and protection, and little dialogue between the Primary and Secondary care services. Conclusion: Overweight is not an outcome based on Primary and Secondary Care, but rather on prescriptive practices, which are not very effective in promoting users’ autonomy. It is necessary to guide the actions taken in these two levels of care to ensure the promotion of effective nutritional care.
