BackgroundObesity can be considered a global public health problem that affects virtually all countries worldwide and results in greater use of healthcare services and higher healthcare costs. We aimed to describe average monthly household medicine expenses according to source of funding, public or private, and to estimate the influence of the presence of obese residents in households on total medicine expenses.MethodsThis study was based on data from the 2008–2009 Brazilian Household Budget Survey, with a representative population sample of 55,970 households as study units. Information on nutritional status and medicines acquired and their cost in the past 30 days were analyzed. A two-part model was employed to assess the influence of obesity on medicine expenses, with monthly household medicine expenses per capita as outcome, presence of obese in the household as explanatory variable, and adjustment for confounding variables.ResultsOut-of-pocket expenses on medicines were always higher than the cost of medicines obtained through the public sector, and 32 % of households had at least one obese as resident. Monthly household expenses on medicines per capita in households with obese was US$ 20.40, 16 % higher than in households with no obese. An adjusted model confirmed that the presence of obese in the households increased medicine expenses.ConclusionObesity is associated with additional medicine expenses, increasing the negative impact on household budgets and public expenditure.

BackgroundIn the last few decades, health systems research (HSR) has garnered much attention with a rapid increase in the related literature. This study aims to review and evaluate the global progress in HSR and assess the current quantitative trends.MethodsBased on data from the Web of Science database, scientometric methods and knowledge visualization techniques were applied to evaluate global scientific production and develop trends of HSR from 1900 to 2012.ResultsHSR has increased rapidly over the past 20 years. Currently, there are 28,787 research articles published in 3,674 journals that are listed in 140 Web of Science subject categories. The research in this field has mainly focused on public, environmental and occupational health (6,178, 21.46%), health care sciences and services (5,840, 20.29%), and general and internal medicine (3,783, 13.14%). The top 10 journals had published 2,969 (10.31%) articles and received 5,229 local citations and 40,271 global citations. The top 20 authors together contributed 628 papers, which accounted for a 2.18% share in the cumulative worldwide publications. The most productive author was McKee, from the London School of Hygiene & Tropical Medicine, with 48 articles. In addition, USA and American institutions ranked the first in health system research productivity, with high citation times, followed by the UK and Canada.ConclusionsHSR is an interdisciplinary area. Organization for Economic Co-operation and Development countries showed they are the leading nations in HSR. Meanwhile, American and Canadian institutions and the World Health Organization play a dominant role in the production, collaboration, and citation of high quality articles. Moreover, health policy and analysis research, health systems and sub-systems research, healthcare and services research, health, epidemiology and economics of communicable and non-communicable diseases, primary care research, health economics and health costs, and pharmacy of hospital have been identified as the mainstream topics in HSR fields. These findings will provide evidence of the current status and trends in HSR all over the world, as well as clues to the impact of this popular topic; thus, helping scientific researchers and policy makers understand the panorama of HSR and predict the dynamic directions of research.

Avaliação do treinamento de manipuladores de alimentos de restaurantes comerciais pelo ensaio ATP-bioluminescênciaEvaluation of food handlers training in commercial restaurants by using ATP bioluminescence methodology RESUMO Os programas de treinamentos para manipuladores de alimentos são meios eficazes para transmitir conhecimentos e promover melhorias nas condições higiênico-sanitárias em estabelecimentos alimentícios. Neste trabalho foi avaliado o efeito do treinamento em boas práticas de manipulação de alimentos, ministrado aos funcionários de restaurantes comerciais da região de Ouro Preto -MG, por meio do teste de ATP-bioluminescência. O teste foi utilizado para quantificar a carga orgânica, medida como valor de "Unidades Relativas de Luz -URL", nas amostras coletadas das mãos de manipuladores, de bancadas de trabalho e de superfícies de utensílios, selecionadas aleatoriamente em cada restaurante. Após o treinamento, houve melhoria no padrão higiênico, evidenciado pela redução da carga orgânica nas superfícies avaliadas em todos os restaurantes. Apesar da redução dos valores de URL, algumas superfícies não alcançaram o valor recomendado para superfície em condições higiênicas satisfatórias. A aplicação do teste ATPbioluminescência para avaliação do treinamento de manipuladores mostrou ser eficaz e pode ser empregado em associação com os tradicionais métodos de contagem de micro-organismos em treinamentos realizados em Unidades de Alimentação e Nutrição. Apesar da redução dos valores de URL indicar a melhoria nas condições higiênicas, o trabalho de treinamento e monitoramento deve ser continuado. ABSTRACTTraining programs designed for food handlers are an effective way to spread the knowledge and to promote improvements on the hygienic conditions of food establishments. This study aimed at evaluating the effect of a training course on good handling practices for food handlers of selected restaurants in the region of Ouro Preto, MG, Brazil, by using ATP-bioluminescence assay. This test was used to quantify the organic load, measured by "Relative Light Units -RLU", in samples collected from workers hands, surfaces of work benches and utensils, which were randomly selected in each restaurant. After training, a significant improvement in the hygienic conditions was observed, as evidenced by the decrease in the RLU values in samples from all restaurants. Despite the decreased RLU values, some surfaces did not reach the recommended value for surfaces samples in the satisfactory hygienic conditions. The ATPbioluminescence test showed to be effective for evaluating the training success, and could be a powerful tool when applied in training programs at food services, and in association with the conventional microbiological methodologies. In spite of the reduction in RLU values, indicating improvements in hygienic conditions, the training and monitoring programs should be continued.
