Resumo: A dor no ombro normalmente pode ser confundida com a dor no pescoço devido ao envolvimento muscular semelhante entre as duas regiões. A relação entre o nível de incapacidade cervical e a dor no ombro relatada por alguns pacientes pode ser encontrada partindo de análises das atividades eletromiográficas dos músculos que representam a musculatura superficial do pescoço. Essa relação também pode ser observada com aplicação de exercícios específicos e de questionários sobre a percepção de incapacidade que o paciente tem ao realizar determinadas tarefas, associando a dor no ombro à incapacidade da coluna cervical. Palavras-chave: ombro, coluna cervical, atividade eletromiográfica. Abstract: Shoulder pain can usually be confused with neck pain due to similar muscle involvement between regions. The relationship between the level of cervical disability and shoulder pain reported by some patients can be found from analyzes of the electromyographic activities of the muscles representing the superficial musculature of the neck. This relationship can also be observed with the application of specific exercises and questionnaires about the patient's perception of disability when performing certain tasks, associating shoulder pain with the incapacity of the cervical spine. Introdução A dor no ombro é uma das queixas mais comuns e incapacitantes do sistema musculoesquelético, ocorrendo em cerca de 40% dos casos da população mundial [1]. Pode ser desencadeada por fatores físicos, como traumas, lesões articulares e capsulares, lesões nas estruturas ligamentares e miotendíneas [2]. Assim como a dor no ombro, a disfunção da coluna cervical é uma condição comum que apresenta um conjunto de sinais e sintomas [3]. A fisiopatologia da dor no ombro e a dor na coluna cervical podem ser diferentes, mas o envolvimento muscular é semelhante e os pacientes podem ter dificuldades em distinguir o local de origem da dor. Isso se deve a conexão biomecânica e anatômica entre a coluna cervical e o complexo do ombro, na qual deficiências que afetam a cervical, bem como alteração na atividade muscular dos flexores cervicais, podem ser associadas com a dor no ombro [4,5]. O presente estudo busca, então, estabelecer uma relação entre o nível de incapacidade no pescoço com a atividade eletromiográfica da musculatura dessa região em pacientes que apresentam dor no ombro. Materiais e métodos Participantes-Houve a participação de 37 indivíduos (13 homens e 24 mulheres) com dor e incapacidade no ombro, com idade variando de 35 a 75 anos. Os critérios de inclusão foram dor no ombro há pelo menos três meses, limitação funcional e resultados positivos para pelo menos três dos seguintes testes de impacto: Hawkins-Kennedy, Neer, Empty can, pain arc e external rotation resistance [6]. Reverse shoulder arthroplasty (RSA), in which anatomic concavities of glenohumeral (GH) joint are inverted, is a popular treatment of arthritic shoulders with deficient rotator cuff. The correct positioning of the glenohumeral center of rotation and initial setting of the deltoid length (Deltoid Tension) plays an important role in the outcome of the RSA. A study of the key literature has shown that despite common use of RSA, its biomechanical characteristics during motion are not fully understood. This study investigates the influence of some of the key parameters on the intensity of the moment in a shoulder after RSA during abduction in scapular plane. The kinematics after RSA are then compared with the anatomic shoulder kinematics and differences are discussed. Mathematical models of both the anatomical and reverse shoulder (RS) were developed in MATLAB and in MSC ADAMS. The anatomical and RSA geometries were defined using measurements obtained from X-ray and magnetic resonance imaging (MRI) images of the shoulder girdle. The results show that in RSA, the intensity of the moment generated in the GH joint improves. However, this improvement does not show a constant trend and its intensity can dramatically decrease in higher GH joint abduction.
