Objective: Assess the relationship between psychiatric relapses of patients with a diagnosis of schizophrenia, the levels of expressed emotion among their relatives and related factors. Method: Prospective study carried out at a mental health outpatient clinic and two Psychosocial Care Centers, with patients and relatives responding to the Family Questionnaire -Brazilian Portuguese Version, a form containing socio-demographic and clinical variables and a structured script to assess relapses. A logistic regression model was used for the analysis. Results: A total of 89 dyads participated in the study. Of the patients investigated, 31% presented relapses and, among the relatives, 68% presented elevated levels of expressed emotion. The relationship between expressed emotion and the relapses was not significant. The logistic regression analysis demonstrated that when there were a higher number of hospital admissions in the two years preceding the study, the chance of the patient relapsing in the 24-month period is 1.34. Conclusion: Expressed emotion was insufficient to predict relapses. Thus, a relapse should be understood as a multifactorial phenomenon. These results provide support for interventions and investigations on the multiple factors involved in the evolution of schizophrenia patients in follow-up at community-based health services. DESCRIPTORS INTRODUCTIONThe concept of Expressed Emotion (EE) has long been used to investigate the family environment of patients with mental disorders. EE reflects the extent to which family members close to a patient express critical, hostile and emotionally over-involved attitudes or warmth when talking about the patient (1) . The main components of EE are critical comments (CC), which are related to negative judgment of patient conduct; hostility (H), related to the negative concept of the patient as a person; and emotional overinvolvement (EOI), which refers to feelings or attitudes, to despair, to self-sacrifice and to overprotection of the patient on the part of the family members. It is worth highlighting that hostility overlaps with critical comments. The family may be classified as having high EE if the family member that spends most of the time with the patient presents one or more of these components (2)(3) . Studies show that EE is a strong predictor of psychiatric relapses in schizophrenic patients, in different social and cultural contexts (4)(5) . A meta-analysis identified 27 articles reporting EE and psychiatric relapses in schizophrenia patients. These studies confirmed that EE is a good predictor of schizophrenia relapses, especially in patients in the most chronic phase of the disease (6) . Este estudo teve como objetivo analisar os diferentes obstáculos e dificuldades que uma família enfrenta quando possui um portador de esquizofrenia no lar, e como a enfermagem pode colaborar com o tratamento e melhoria na qualidade de vida dos mesmos. Trata-se de um estudo exploratório, descritivo, do tipo revisão integrativa da literatura (RIL). Para coleta de dados foram utilizadas as bibliotecas virtuais de pesquisa: SciELO, LILACS, PubMed e Google Acadêmico, aplicando critérios de elegibilidade e inelegibilidade para seleção das publicações. Nos resultados mostraram que familiares que convivem com portadores de esquizofrenia possuem dificuldades para entender o distúrbio, identificar e lidar diante as crises gerando uma certa sobrecarga, tanto ao paciente quanto ao cuidador, fazendo-se necessário que a equipe de enfermagem acolha essas famílias fornecendo informações sobre o distúrbio, estabelecendo um impacto positivo nos familiares e paciente, consequentemente gerando melhora para o paciente e família, pois é extremamente importante um convívio social saudável para melhora do quadro clinico do paciente com esquizofrenia, junto ao tratamento com a equipe multidisciplinar. Conclui-se que a relação entre a equipe de enfermagem e os familiares dos portadores de esquizofrenia é compartilhada por meio dos sentimentos e emoções gerados pela experiência entre eles. Ao fornecer informações e realizar o acolhimento, a equipe de enfermagem pode estabelecer uma relação que tem impacto positivo nos familiares, podendo ser uma das consequências da melhoria do cuidado ao paciente. Por meio dos profissionais de enfermagem, é possível refletir sobre as inserções, movimentos e interações estabelecidas entre eles e o paciente esquizofrênico.

Introduction: Schizophrenia, object of this review, is a chronic degenerative mental disease. It occupies the top position of the mental illness list, being considered the most serious mental disorder, with significant repercussions on public health. Schizophrenia is known as a deteriorating phenomenon, which causes damages on the life of the affected person and his family. Goals: To analyze the scientific literature about schizophrenia, in the period between 1983 and 2015, published at REBEn. Methods: This is an integrative literature review. The literature research was made in the journal REBEn, which can be found on the following databases: Lilacs, Nursing Index, CINAHL and Scielo. The research included original articles with full text available in electronic support. The descriptors used were: schizophrenia, mental suffering and nursing. Seven scientific articles were selected and analyzed. Results: In 2005 and 2012, the higher number of publications could be found. Regarding the authorship of the articles, most had the participation of masters and doctors. Most of the researched production was concentrated in the Southeast Region, with unanimous qualitative approach. Hospitals were the biggest scenario of this research. Conclusion: There are few studies published in Nursing about schizophrenia, as evidenced by the decreasing of production about it for 32 years of publication of REBEn. The scientific productions proved to be important to change behaviors on nursing practice, however, there is a need for more studies on this subject.
