OBJECTIVE Evaluate the association of multimorbidity, primary health care model and possession of a private health plan with hospitalization.METHODS A population-based cross-sectional study with 1,593 elderly individuals (60 years old or older) living in the urban area of the city of Bagé, State of Rio Grande do Sul, Brazil. The outcome was hospitalization in the year preceding the interview. The multimorbidity was evaluated through two cut-off points (≥ 2 and ≥ 3). The primary health care model was defined by residence in areas covered by traditional care or by Family Health Strategy. The older adults mentioned the possession of a private health plan. We performed a gross and adjusted analysis by Poisson regression using a hierarchical model. The adjustment included demographic, socioeconomic, functional capacity disability and health services variables.RESULTS The occurrence of overall and non-surgical hospitalization was 17.7% (95%CI 15.8–19.6) and 10.6% (95%CI 9.1–12.1), respectively. Older adults with multimorbidity were admitted to hospitals more often when to older adults without multimorbidity, regardless of the exhibition’ form of operation. Having a private health plan increased the hospitalization by 1.71 (95%CI 1.09–2.69) times among residents in the areas of the Family Health Strategy when compared to elderly residents in traditional areas without a private health plan.CONCLUSIONS The multimorbidity increased the occurrence of hospitalizations, especially non-surgical ones. Hospitalization was more frequent in older adults with private health plan and those living in Family Health Strategy areas, regardless of the presence of multiple diseases.

RESUMOEste estudo teve como objetivo descrever o perfil de consumo de medicamentos e identificar fatores associados entre adultos que aguardavam por atendimento na sala de espera de três unidades de Estratégia de Saúde da Família (ESF) do município de Santa Rosa-RS. Trata-se de um estudo transversal, analítico e descritivo. A coleta de dados foi realizada durante três semanas no mês de agosto de 2014, uma semana em cada unidade. Os dados obtidos foram analisados estatisticamente através do teste Anova, seguido de Test T de Student para as amostras independentes. Identificou-se entre os entrevistados que 58,95% utilizam medicamentos de forma contínua, com média de 2,08 medicamentos/indivíduo. Entre as variáveis relacionadas ao uso contínuo de medicamento observaram-se a faixa etária, acima de 50 anos, presença de doença crônica, escolaridade e frequência de uso dos serviços da unidade de saúde. Entre as variáveis que não apresentaram relação identificaram-se prevalência de mulheres (86,03%), uso correto dos medicamentos (83,84%), o recebimento de orientações para o uso de medicamentos (96,07%), os que referiram cuidar da alimentação (73,36%) e o sedentarismo (71,62%). Garantir acesso a medicamentos encontrase entre as diretrizes do SUS, porém, é responsabilidade do serviço de saúde promover a utilização desses produtos de forma eficaz e segura. Como estratégia para estes objetivos apresenta-se a sensibilização e capacitação das equipes de saúde e o incentivo à corresponsabilização do usuário de medicamentos sobre o seu tratamento, processo que pode ser desenvolvido através da educação em saúde e do matriciamento, sendo fundamental a participação ABSTRACT This study's objective is to describe the medication use profile and identify factors associated with medication use among adults waiting for medical care in the waiting rooms of three units of the Family Health Strategy (FHS) in the city of Santa Rosa, RS. This is a cross-sectional, analytical, and descriptive study. Data collection was carried out over three weeks in August 2014, one week in each unit. Data were statistically analyzed using ANOVA followed by the Student's t-test for the independent samples. It was observed that 58.95% of those interviewed use medication continuously, with a mean use of 2.08 medications/individual. Among the variables correlated with continuous drug use, we observed: the over-50 age group, chronic disease, educational level, and frequency of use of the health unit services. Among the variables with no observed correlation, we identified: a prevalence of women (86.03%), correct use of medications (83.84%), having received guidance on the use of medications (96.07%), those who reported healthy eating (73.36%), and sedentary lifestyle (71.62%). RESUMOObjetivou-se com este estudo, compreender os motivos da não realização da mamografia por mulheres idosas. Pesquisa qualitativa realizada com 13 idosas entre 60 e 69 anos, que nunca realizaram a mamografia. A coleta de dados foi realizada por meio de quatro grupos focais. Os resultados foram organizados em categorias temáticas e discutidos à luz da literatura. As idosas revelaram que entre os motivos para não realizar a mamografia encontra-se a dificuldade de acesso aos serviços de saúde, as crenças relacionadas ao cuidado com as mamas, o constrangimento e o medo do desconforto durante a realização do exame. Considerando-se as barreiras institucionais e pessoais, que limitam a realização da mamografia, ressalta-se a necessidade de ações profissionais que possam aumentar a utilização desse exame para o rastreamento do câncer de mama. INTRODUÇÃOO câncer de mama (CM) é o segundo tipo de neoplasia mais incidente em mulheres em todos os países, perdendo espaço apenas para os tumores de pele não melanoma. 1 A estimativa de incidência de CM para o Brasil, nos anos de 2014 e 2015, foi de 57.120 casos, com risco estimado de 52 casos a cada 100 mil mulheres a cada ano. 2 ABSTRACTThis study aimed to understand the reasons for not doing a mammography among older women. Qualitative research was conducted with 13 elderly women between 60 and 69 years of age who had never done a mammography. Data collection was carried out through four focus groups. The results were organized into thematic categories and discussed considering the literature. These women revealed that among the reasons for not doing a mammography there is the difficulty in access to health services, beliefs related to the care of the breasts, and the embarrassment and fear of discomfort during the exam. Considering the institutional and personal barriers that keep women from undergoing mammography, the need is emphasized for professional actions that can increase the use of mammography to screen for breast cancer.
