Objective: To track symptoms of posttraumatic stress disorder (PTSD) according to scores of the Impact of Event Scale-Revised (IES-R) in nursing staff exposed to accidents with biological material, from October 2014 to May 2016, in a philanthropic hospital in the state of Minas Gerais. Methods: Retrospective, descriptive study with a quantitative approach. Inferential analysis was performed using a 95% confidence interval, p ≤ 0.05. The data were collected considering all the ethical aspects described in resolution 466/2012. Results: The majority of workers were nursing technicians (52.5%), female (91.8%), with a mean age of 31.4 years (SD 7.5), the majority of exposures being percutaneous (60.7%). Of all individuals, 19.6% presented scores above 5.6, showing signs of PTSD. Individuals who had suffered accidents more than six months previously (p = 0.025) had significantly higher scores than those who had been exposure for less than three months. Conclusion: The study highlights the need for the use of validated instruments for the evaluation of fear, anxiety, stress, and other psychological reactions that characterize PTSD, as well as the provision of psychological care for workers exposed to biological material. Las exposiciones se produjeron por vía percutánea (60,7%). Del total de individuos, el 19,6% presentó escores por encima de 5,6 evidenciando señales para TEPT y aquellos que se habían accidentado hace más de seis meses tuvieron puntuaciones significativamente superiores (p = 0,025) a los que sufrieron exposición hace menos de tres Los meses. Conclusión: Se evidenció la necesidad del uso de instrumentos validados para la evaluación del miedo, ansiedad, estrés entre otras reacciones psicológicas que caracterizan el trastorno de estrés postraumático y de la oferta de atención psicológica para los trabajadores expuestos a material biológico. rESUMo objetivo: Identificar a incidência de transtorno de estresse pós-traumático (TEPT) em pacientes submetidos à ventilação mecânica (VM) e analisar a sua relação com variáveis clíni-cas, demográficas e presença de recordações referentes à unidade de terapia intensiva (UTI). Método: Estudo prospectivo longitudinal realizado durante o período de um ano na UTI de um hospital-escola. A avaliação ocorreu três meses após a alta da UTI durante consulta ambulatorial. Os sujeitos foram avaliados utilizando-se um instrumento para avaliar memórias de UTI (IAM-TI) e o Impact of Event Scale-Revised (IES-R), para avaliar o TEPT. resultados: Do total de respondentes, 68,8% eram do sexo masculino, a idade variou entre 18 e 79 anos, com média de 43,5 anos ± 17,1. Quanto à avaliação dos pacientes, 14,5% apresentaram sintomas de TEPT, quando utilizado um ponto de corte > 20 pontos. Para uma melhor precisão diagnós-tica, foi utilizado também o ponto de corte ≥ 33 pontos e, nesse caso, sete pacientes (5,1%) apresentaram o transtorno. Conclusão: Poucos pacientes preencheram todos os critérios para o diagnóstico de TEPT, indicando que a grande maioria deles apresentou apenas sintomatologia relacionada ao TEPT. aBStraCt objective: Identify the incidence of post-traumatic stress disorder (PTSD), in patients submitted to MV and analyze its relation with clinical and demographical variables, and with the presence of memories regarding intensive care unit (ICU). Method: It is a longitudinal prospective study, carried out in one year at an ICU of a university hospital. Evaluation occurred three months after ICU discharge during patients follow up at the ambulatory. Subjects were evaluated using the ICU memories (ICU-AMI) and the Impact of Event Scale-Revised (IES-R) instruments. results: From the total of subjects, 68.8% were male from 18 to 79 years of age, 43.5 years old average ± 17.1. From the evaluated patients, 14.5% presented symptoms of PTSD when a cut-off score > 20 was used. For a diagnostic accuracy a ≥ 33 cut-off score was also used, and in this case, only seven patients (5.1%) showed signs of PTSD. Conclusion: A few patients met the criteria for the PTSD diagnosis indicating that the great majority of patients admitted at ICU show only symptomatology related to PTSD.

Association between acute disease severity and one-year quality of life among post-hospitalisation COVID-19 patients: Coalition VII prospective cohort study

Purpose To assess the association between acute disease severity and 1-year quality of life in patients discharged after hospitalisation due to coronavirus disease 2019 (COVID-19). Methods We conducted a prospective cohort study nested in 5 randomised clinical trials between March 2020 and March 2022 at 84 sites in Brazil. Adult post-hospitalisation COVID-19 patients were followed for 1 year. The primary outcome was the utility score of EuroQol five-dimension three-level (EQ-5D-3L). Secondary outcomes included all-cause mortality, major cardiovascular events, and new disabilities in instrumental activities of daily living. Adjusted generalised estimating equations were used to assess the association between outcomes and acute disease severity according to the highest level on a modified ordinal scale during hospital stay (2: no oxygen therapy; 3: oxygen by mask or nasal prongs; 4: high-flow nasal cannula oxygen therapy or non-invasive ventilation; 5: mechanical ventilation). Results 1508 COVID-19 survivors were enrolled. Primary outcome data were available for 1156 participants. At 1 year, compared with severity score 2, severity score 5 was associated with lower EQ-5D-3L utility scores (0.7 vs 0.84; adjusted difference, − 0.1 [95% CI − 0.15 to − 0.06]); and worse results for all-cause mortality (7.9% vs 1.2%; adjusted difference, 7.1% [95% CI 2.5%–11.8%]), major cardiovascular events (5.6% vs 2.3%; adjusted difference, 2.6% [95% CI 0.6%–4.6%]), and new disabilities (40.4% vs 23.5%; adjusted difference, 15.5% [95% CI 8.5%–22.5]). Severity scores 3 and 4 did not differ consistently from score 2. Conclusions COVID-19 patients who needed mechanical ventilation during hospitalisation have lower 1-year quality of life than COVID-19 patients who did not need mechanical ventilation during hospitalisation.
