Determinantes da mortalidade neonatal: estudo caso-controle em Fortaleza, Ceará, Brasil

Abstract: (OR = 0.23;, time spent from home to the hospital ≥ 30 minutes (OR = 3.12; (OR = 14.75; prematurity (OR = 3.41; IntroduçãoA partir da década de 90, em virtude da redução do componente pós-neonatal da mortalidade infantil, aumentou o peso relativo às mortes ocorridas no período neonatal. Na segunda metade da década de 90, a taxa de mortalidade neonatal, em torno de 20 óbitos por mil nascidos vivos, passou a representar mais de 60% da mortalidade infantil 1,2,3 . Percebeu-se uma maior concentração desses óbito… Show more

“…Unlike other studies that have analyzed associated factors to neonatal mortality [26][27][28] in this city, a late onset on prenatal care and the number of prenatal consultation visits did not increase the chance for near miss. 11,17 These controversies indicate gaps in knowledge about the participation to access prenatal care, the quantity and quality of consultations in the determination of negative outcomes for the mother and the newborn, in socioeconomic contexts with great disparities, such as in the case in Brazil.…”

“…18,25,27 Our results show that the variable that most likely associated to neonatal near miss morbidity was the classification of risk to live births according to the Program for Child Health ( Programa de Sáude da Criança) in the city. This finding may be plausible since the risk criteria adopted by the program understand the factors already established for infant morbimortality, especially neonatal.…”

“…Very low birth weight, gestational age of less than 32 weeks and a low Apgar score 15 are lifethreatening conditions associated with a high risk of neonatal death 14 . The use of mechanical ventilation is typically associated with serious respiratory disease.…”

“…This situation has been termed the 'Brazilian perinatal paradox': deliveries and births are intensely medicalized, but higher rates of maternal and perinatal morbidity and mortality persist, and are possibly related to poor quality of care and use of obsolete and iatrogenic procedures, which may affect perinatal outcomes 9,10 . One of the most prominent examples in that respect is the high rate of caesarean sections in Brazil -53.7% of births in 2011 1 .…”
