-Background -Crohn's disease and ulcerative colitis are the two major forms of inflammatory bowel disease. The incidence and prevalence of both conditions have increased and are progressively increasing. These diseases are frequently recurrent and clinically highly severe. In Brazil, the lack of epidemiological data related to such diseases has left these patients in a vulnerable state and contributed to increased morbidity. Objective -To describe the profiles of patients with inflammatory bowel disease treated in an outpatient service in Brazil. Methods -This descriptive, exploratory, and retrospective documentary study with a quantitative approach was performed in an outpatient treatment service for inflammatory bowel disease, at a university polyclinic located in Rio de Janeiro, Brazil, from May to July 2016. The study included 556 patients and was approved by the research ethics committee of the institution (CAAE no. 55179316.6.0000.5259/2016). Results -The data showed a high prevalence of inflammatory bowel disease in white female patients. Crohn's disease was diagnosed in more patients than was ulcerative colitis; the ileocolon was the most commonly affected location in patients with Crohn's disease. The stenotic phenotype was prevalent in patients with Crohn's disease. Conclusion -The prevalence of the stenotic phenotype in Crohn's disease in relation to others demonstrates the need for further investigations in this field of study in Brazil. In conclusion, the data showed that the epidemiologic profile of the study population is similar to that published in the national and international literature. HEADINGS -Inflammatory bowel diseases. Esta entidade comumente encontra-se acompanhada de manifestações extra-intestinais, tais como eritema nodoso, pioderma gangrenoso, artrite, uveíte, colangite esclerosante e câncer colorretal. Dentre essas, destacamos a colangite esclerosante que, pelo risco de malignizar para colangiocarcinoma, apresenta altos índices de morbimortalidade em curto período de tempo e, apesar de raro na população em geral, é relativamente comum em pacientes com RCU. Trabalhos recentes mostram que cerca de 5% dos pacientes com RCU são acometidos pela colangite esclerosante e desses, 10 a 15% desenvolvem o colangiocarcinoma. 3,4 Pacientes com RCU e colangiocarcinoma apresentam um quadro clínico caracterizado pela presença de massa hepática expansiva, podendo haver sintomas obstrutivos, febre, suor noturno, perda de peso e dor em quadrante superior direito do abdômen. O tratamento indicado varia de acordo com o grau da malignização, podendo-se realizar a ressecção do tumor, quimioterapia adjuvante, além do tratamento sintomático. Até o momento o tratamento cirúrgico é o que oferece melhores chances de cura. 5

Resultados: Participaram do estudo 122 pacientes de ambos os sexos com doença inflamatória intestinal, estratificados em: doença de Crohn (n=64) e retocolite ulcerativa (n=58). As mulheres representaram 54,1% da amostra, a média de idade foi de 41,6±12,6 anos e a média de índice de massa corporal foi de 25 ± 4,4kg/m 2 . O colesterol ligado à lipoproteína de baixa densidade (LDL-c) mostrou-se significativamente mais elevado nos pacientes com retocolite ulcerativa comparando-se aos pacientes com doença de Crohn (p=0,006). A proteína C reativa aumentada esteve associada com maior gravidade do processo inflamatório em pacientes com doença de Crohn (p=0,027), quando analisada pelo índice de Harvey-Bradshaw. Na retocolite ulcerativa, conforme o índice de Truelove e Witts, houve correlação fraca entre atividade da doença e colesterol ligado à lipoproteína de alta densidade (HDL-c) (r s = -0,29; p=0,036) e entre atividade da doença e colesterol total (r s = -0,30; p=0,028). Cerca de dois terços dos pacientes (67,6%) apresentaram o HDL-c abaixo dos valores recomendados. Conclusões: O LDL-c mostrou-se significativamente mais elevado nos pacientes com retocolite ulcerativa comparados aos pacientes com doença de Crohn. O HDL-c apresentou níveis reduzidos em pacientes com doença inflamatória mais grave na retocolite ulcerativa. A proteína C reativa aumentada em pacientes com doença de Crohn esteve associada com a gravidade do processo inflamatório determinada pelo índice de Harvey-Bradshaw. A detecção dessas alterações metabólicas pode auxiliar na identificação de indivíduos com potencial risco para eventos cardiovasculares. ABSTRACT Aims:To evaluate the lipid profile and inflammatory parameters in patients with Crohn's disease (CD), and ulcerative colitis (UC) and correlate them with disease activity levels. Methods: Cross-sectional study of patients followed up at the Inflammatory Bowel Disease Outpatient Clinic of São Lucas Hospital at Pontifícia Universidade Católica do Rio Grande do Sul, Brazil. The demographic, anthropometric, and metabolic characteristics of patients with chronic inflammatory disease were evaluated. Lipids and inflammatory parameters were evaluated as well as the correlation between the severity of inflammatory disease and metabolic parameters using the Harvey-Bradshaw index for CD and Truelove and Witts' method for UC.
