RESUMO ABSTRACTA retrospec ve, cross-sec onal study of 30 women seen at a Basic Health Care Unit (BHCU) in the Eastern Sec on of São Paulo, using interviews and medical record reviews, to determine how group B streptococcus (GBS) was detected in pregnant women. The pa ents in the studied sample received prenatal care at the UBS and delivered their babies between January 2009 and December 2010. Twenty-three of the women (76.7% of the total) underwent a GBS culture, 82.6% of which yielded a nega ve result and 17.4% of which yielded a posi ve result; 43.5% of these women underwent the test between 35 and 37 weeks of gesta on; and 23.5% of the parcipants were not tested, mostly because no test was requested. RESUMENEstudio transversal y retrospectivo, realizado con 30 mujeres pacientes de una Unidad Básica de Salud (UBS) de la Zona Este de São Paulo mediante entrevista y consulta de historias clínicas, con el objetivo de verificar como se efectúa la investigación de estreptococos del grupo B en gestantes. La muestra realizó prenatal en la UBS y tuvo sus bebés en el período de enero de 2009 a diciembre de 2010. Veintitrés mujeres (76,7% del total) realizaron la cultura de EGB, 82,6% con resultados negativos y 17,4% positivos; 43,5% de las pacientes realizaron el examen entre las semanas 35 y 37 de gestación; 23,5% no realizaron el examen, la mayor parte por no haber efectuado la solicitud. Fue posible verificar que ocurrieron fallas en el rastreo del EGB durante el período seleccionado.

BackgroundA valid, accurate method for determining gestational age (GA) is crucial in classifying early and late prematurity, and it is a relevant issue in perinatology. This study aimed at assessing the validity of different measures for approximating GA, and it provides an insight into the development of algorithms that can be adopted in places with similar characteristics to Brazil. A follow-up study was carried out in two cities in southeast Brazil. Participants were interviewed in the first trimester of pregnancy and in the postpartum period, with a final sample of 1483 participants after exclusions. The distribution of GA estimates at birth using ultrasound (US) at 21–28 weeks, US at 29+ weeks, last menstrual period (LMP), and the Capurro method were compared with GA estimates at birth using the reference US (at 7–20 weeks of gestation). Kappa, sensitivity, and specificity tests were calculated for preterm (<37 weeks of gestation) and post-term (>=42 weeks) birth rates. The difference in days in the GA estimates between the reference US and the LMP and between the reference US and the Capurro method were evaluated in terms of maternal and infant characteristics, respectively.ResultsFor prematurity, US at 21–28 weeks had the highest sensitivity (0.84) and the Capurro method the highest specificity (0.97). For postmaturity, US at 21–28 weeks and the Capurro method had a very high sensitivity (0.98). All methods of GA estimation had a very low specificity (≤0.50) for postmaturity. GA estimates at birth with the algorithm and the reference US produced very similar results, with a preterm birth rate of 12.5%.ConclusionsIn countries such as Brazil, where there is less accurate information about the LMP and lower coverage of early obstetric US examinations, we recommend the development of algorithms that enable the use of available information using methodological strategies to reduce the chance of errors with GA. Thus, this study calls into attention the care needed when comparing preterm birth rates of different localities if they are calculated using different methods.

Os objetivos são identificar quais os motivos dos encaminhamentos de gestantes ao pré-natal de alto risco do município de Aracaju-SE no ano de 2016, assim como verificar o índice de gestantes inseridas no programa após o encaminhamento e identificar a faixa etária e idade gestacional dessas gestantes encaminhadas. A pesquisa é de caráter descritivo, documental retrospectivo, com abordagem quantitativa que avaliou 704 encaminhamentos de gestantes ao Pré-Natal de Alto Risco, utilizando os dados da Planilha de Controle da Regulação ao Pré-Natal de Risco do município de Aracaju/SE, durante o período de janeiro a dezembro de 2016. A síndrome hipertensiva foi a patologia mais prevalente, representando 21,59% das patologias mais encaminhadas ao serviço de alto risco. Em relação a faixa etária, o maior percentual de encaminhamentos encontra-se entre 25 a 29 anos (21,73%). Referente a idade gestacional, gestantes com até 22 semanas são encaminhadas com maior frequência ao serviço de maior complexidade. A maioria das gestantes encaminhadas a este serviço é inserida no pré-natal de alto risco (88,78%). Conclui-se que os maiores motivos de encaminhamentos ao pré-natal de alto risco, no município de Aracaju, são por complicações patológicas. Além disso, as gestantes com faixa etária entre 25 e 29 anos e idade gestacional < 22 semanas são as mais encaminhadas a este serviço.
