Divertículo ventricular congênito associado à taquicardia ventricular

Abstract: Congenital ventricular diverticula are rare. Clinically, they may be asymptomatic or cause systemic embolization, heart failure, valvular regurgitation, ventricular rupture, ventricular arrhythmia, or sudden death. We report the case of a 56-year-old woman with sustained ventricular tachycardia, who, during investigation, was diagnosed with a diverticulum in the inferobasal portion of the left ventricle. The clinical characteristics and treatment of this rare disease are discussed.

“…Eighteen percent of all patients had to be rehospitalized during the follow-up period, 73% of them for cardiovascular reasons. If one focuses only on symptoms known to be frequently related to LVA and LVD (embolic events, rhythm disturbances, syncope) [2,10,11,12,13], almost 30% of our study population complained of 1 or more symptoms out of these 3 categories during the follow-up period. The presence of LVD significantly increases the risk of embolic events (p = 0.04).…”

“…Symptomatic patients usually suffer from ventricular arrhythmias, ranging from occasional ventricular premature beats to sudden cardiac death. Patients with documented ventricular tachycardia show right bundle branch block morphology according to the left ventricular origin of the arrhythmogenic substrate [6, 23, 24, 25, 27, 45]. Frequent causes of complaints are supraventricular rhythm disturbances, e.g.…”

“…Depending on its location, blood supply of a diverticulum is derived from the right or left coronary artery, or both, with a normal venous return [18]. Congenital aneurysms mostly show connective tissue with reticulin fibers (Masson staining) and only few unorganized muscular fibers with various degrees of vacuolization [45]. The internal surface is covered with simple endothelium (CD31 and CD34 staining) [2, 18, 25, 36 ](fig.…”

“…6 If not associated with the latter, as was seen in our case, they may present with non-specific abnormalities on resting electrocardiogram, including atrial ectopic beats, ST-T wave changes, and arrhythmias. 7 A fibrous left ventricular diverticulum is pathologically characterised by the presence of only fibrous tissue and no myocardial wall component. These are found to be non-contractile on echocardiography and more prone to rupture in comparison with the other type.…”
