Portuguese / English: www.scielo.br/reeusp RESUMO Com o objetivo de compreender o conhecimento, as preocupações e as expectativas de pacientes portadoras de câncer de mama em relação à mastectomia, entrevistou-se 11 mulheres internadas no pré-operatório da cirurgia, no Hospital das Clínicas de Fortaleza (CE), em dezembro de 2008. O método hermenêutico-dialético foi utilizado na análise dos discursos, sendo definidas três categorias temáticas: Conhecimento acerca da cirurgia, Sentimentos e expectativas em torno da mastectomia e a Retirada da mama. Constatou-se que a mulher passa por um pré-operatório estressante, com desconhecimento acerca da cirurgia, sente ansiedade e medo pelo que está por vir, além de sentimentos de pâni-co e choque diante da retirada da mama. Ressalta-se, portanto, a importância do papel educativo e do apoio emocional direcionados por toda a equipe que presta assistência a essas pacientes, de maneira a minimizar o drama que vivenciam. DESCRITORES ABSTRACTThe objective of this study was to understand the knowledge, concerns and expectations of patients suffering from breast cancer in relation to mastectomy. Interviews were performed with 11 women before their being submitted to surgery at the Hospital das Clínicas in Fortaleza (Ceará state) on December 2008. The hermeneutic-dialectical method was used for discourse analysis, and three theme categories were defined: Knowledge about the surgery, Feelings and expectations surrounding mastectomy, and Removing the breast. It was found that women go through a stressful preoperative period, lack knowledge about the surgery, experience anxiety and fear of what they should expect, as well as feelings of panic and shock before their breast is removed. It is, therefore, observed that the health team caring for those patients has an important role in education and in providing emotional support so as to minimize the tragedy those women are experiencing. KEY WORDS RESUMENCon el objetivo de entender el conocimiento, las preocupaciones y expectativas de pacientes portadoras de cáncer de mama en relación a la mastectomía, fueron entrevistadas en el preoperatorio de la cirugía 11 mujeres internadas en el Hospital de Clí-nicas de Fortaleza-Ceará-Brasil, en el mes de diciembre de 2008. El método hermenéutico-dialéctico fue utilizado en el análi-sis de los discursos, definiéndose tres categorías temáticas: Conocimiento acerca de la cirugía, Sentimientos y expectativas en torno a la mastectomía y Extirpación mamaria. Se constató que la mujer pasa por un preoperatorio estresante, con desconocimiento al respecto de la cirugía, ansiedad y miedo de lo que está por venir, además de sentimientos de pánico y shock ante la extirpación de la mama. Se resalta, por lo tanto, la importancia del papel educativo y el apoyo emocional brindados por todo el equipo que atiende a tales pacientes, de manera tal de minimizar el drama que experimentan. ReSUmO Trata-se de uma pesquisa bibliográfica realizada nas bases de dados Lilacs e Scielo, que objetivou revisar a produção científica referente aos cuidados de enfermagem no período pré-operatório e de reabilitação de mastectomia. Foram analisados dezoito artigos focalizados nas seguintes categorias: Cuidados de enfermagem no pré-operatório da mastectomia e O papel da Enfermagem na reabilitação da mulher mastectomizada. Observou-se que os cuidados de enfermagem nos dois períodos são fundamentais para um tratamento mais humanizado. No entanto, estes se encontram escassos e com orientações pouco esclarecedoras, sendo necessário um maior enfoque por parte da equipe com relação ao seu papel e cuidados prestados à mulher nessa condição. descritores: Enfermagem; Cuidados de enfermagem; Neoplasias das mamas; Mastectomia. ABSTRACT This is a bibliographic research carried out in Lilacs and Scielo database, which aimed to review the production about nursing care during the preoperative and rehabilitation stages of mastectomy. Eighteen articles were analyzed with focus in these categories: Nursing care in the preoperative mastectomy and The role of nursing in the rehabilitation of women submitted to a mastectomy. It was observed that nursing cares, in the two periods, are a key step for a more human treatment. RESUMEN Este estudio da cuenta de una revisión de la literatura de la atención de enfermería en el pre-operatorio bien como en la rehabilitación de la mastectomía. Fueron analizados dieciocho artículos de acuerdo con las categorías: Cuidados de enfermería en el preoperatorio de mastectomía y El papel de la enfermería en la rehabilitación de las mujeres la mastectomía. Se observó que la atención de enfermería en ambos períodos es crucial para una asistencia más humanizada. Objective: The coronary artery bypass graft surgery is an option of treatment for cardiovascular diseases, and the patients who underwent such procedure can suffer from preoperative anxiety. The aim of this study is to verify the effects of physiotherapeutic instructions at the anxiety level on patients undergone coronary artery bypass graft surgery.Methods: Randomized clinical trial, with the sample composed by 51 individuals, 27 on the control group and 24 on the intervention; on which it was assessed the anxiety (Beck Scale for Anxiety) and pain (Visual Analog Scale) levels on the pre-and postoperative period and only the second group received instructions about the surgery procedures and ventilatory exercises. For statistical analysis the Wilcoxon, Mann-Whitney and Spearman tests were used.Results: The lower anxiety levels were observed in patients that received the intervention on the period before surgery (9.6 ± 7.2 versus 13.4 ± 5.9, P=0.02). On the control group, the difference among the anxiety levels before and after the surgery was statistically significant (P= 0.003). The female individuals presented more anxious before the surgery compared to the male ones (P=0.058). It was also verified a relationship between lenght of stay at hospital and postoperative anxiety (P=0.05), where the more anxious individuals remained more time at the hospital.Conclusion: Patients oriented and instructed about physiotherapeutic ventilatory exercises and hospital routine, presented their levels of anxiety reduced on the preoperative compared to the control group. However, at the postoperative, both groups presented their anxiety levels reduced without significant difference between them. Descriptors: Anxiety. Myocardial revascularization.
