Pneumoventricle and liquoric fistula are possible complications of traumatic brain injury (TBI), the main cause of morbimortality related to trauma in Brazil. Liquoric fistulae are more common after direct trauma with skull base fractures. However, pneumoventricle is rare and occurs due to excessive cerebrospinal fluid (CSF) drainage in the presence of a poorly compliant ventricle system, resulting in the influx of air to its interior. The pathophysiology of tension pneumoventricle remains uncertain. However, the traumatic cause is certain and multiple bone fractures of the face and liquoric fistula may contribute to the process. If symptomatic, the tension pneumoventricle can cause rapid clinical deterioration. The authors aim to report a rare case of post-TBI tension pneumoventricle with complete resolution and without signs of recurrence of the liquoric fistula after surgical treatment. Palavras-Chave ResumoO pneumoventrículo e a fístula liquórica são possíveis complicações decorrentes do traumatismo cranioencefálico (TCE), principal causa de morbimortalidade relacionada ao trauma no Brasil. As fístulas liquóricas são mais comuns após traumas diretos, com fraturas da base do crânio. Já o pneumoventrículo é raro e ocorre após a drenagem liquórica excessiva, na presença de sistema ventricular pouco complacente, resultando em um influxo de ar para o seu interior. A fisiopatologia do pneumoventrículo hipertensivo permanece incerta. Entretanto, a causa traumática é certa, e múltiplas fraturas de ossos da face e a fístula liquórica podem ter contribuição no processo. Se for sintomático, o pneumoventrículo hipertensivo pode provocar rápida deterioração clínica. Os autores têm por objetivo relatar um caso raro de pneumoventrículo hipertensivo após TCE com completa resolução e sem sinais de recorrência da fístula liquórica após tratamento cirúrgico.

Introdução: Um dos principais problemas de saúde pública de âmbito mundial, o Traumatismo Crânio Encefálico (TCE) está se tornando cada vez mais incidente no mundo moderno. O traumatismo crânio-encefálico (TCE) é um ataque causado por fatores físicos ao crânio decorrente de impacto externo, podendo ser penetrante ou não, gerando modificações cerebrais como: incapacidade intelectual, transtornos de mobilidade física e de cognição, seja momentânea ou irreversível. Objetivo: O presente estudo tem intuito de apresentar uma revisão bibliográfica caracterizando a produção científica acerca da assistência de enfermagem, expondo quais principais intervenções de enfermagem diante de um paciente vítima de TCE. Metodologia: Trata-se de uma revisão integrativa realizada no período entre 2010 a 2017, indexados nas bases de dados: LILACS, SCIELO, BDENF e MEDLINE. Resultado: Identificar corretamente as principais necessidades básicas individuais do paciente possibilitando ao profissional enfermeiro alcançar objetivos que representam progresso na assistência à saúde como também na inovação de possibilidades para conduzir o atendimento e a terapêutica ao cliente, ponderando a diminuição de sequelas e resolução final. Conclusão: Para que se garanta uma assistência de forma qualificada e humanizada aos pacientes vítimas de TCE, é importante que a equipe de enfermagem esteja apta a desempenhar sua função, dando ênfase a uma sistematização de cuidados que garantem a autonomia de enfermagem na equipe multiprofissional. Essa prestação de cuidados ao paciente exige do profissional enfermeiro múltiplos conhecimentos e a compreensão e iniciativa quanto ao processo de liderança da equipe, destacando o relacionamento interpessoal com familiares das vitimas e tomada de decisões eficazes.

Introduction: Neuraxial techniques, including epidural anaesthesia, are often used for perioperative pain control and are generally safe. However, both transient, mild and even severe, life-threatening neurologic complications can occur. Case presentation: A seventy-eight-year-old man was admitted to the hospital for a radical nephrectomy plus transurethral resection due to kidney and bladder cancer. During the epidural exploration, an accidental dural puncture was noted. This was followed by the patient complaining of an intense headache. The epidural catheter was placed in a different location, and surgery was performed uneventfully. The patient presented with confusion, agitation, vertical nystagmus, vision loss, and paraparesis about two hours later. The epidural levobupivacaine and morphine infusion were stopped, followed by motor block resolution. A computerized head-tomography scan showed extra-axial multiple air spots involving the frontal and temporal lobes. Emergent hyperbaric oxygen therapy was commenced. After a single session, there was complete resolution of all symptoms and a marked reduction in the number and volume of the extra-axial air visible on the CT scan. Conclusions: Although rare, pneumocephalus is a well-recognized complication of a dural puncture. Its rapid recognition in a patient with new-onset neurological symptoms and early treatment with hyperbaric oxygen therapy allows rapid clinical and imaging resolution and an improved prognosis.

Abstract: Pneumocephalus after accidental perforation of the dura mater presented headache with the characteristics of headache secondary to loss of CSF, but with spontaneous resolution after the air was absorbed. Invasive measures, such as epidural blood patch, were not necessary.
