In this case, sugammadex was used to reverse the hemodynamic picture caused by rocuronium-induced anaphylaxis. However, it was not possible to identify the exact mechanism for the reversal.
RESUMOBarbosa FT, Cunha RM, Barbosa LT -Doença de von Willebrand e Anestesia.JUSTIFICATIVA E OBJETIVOS: A doença de von Willebrand ocorre devido à mutação no cromossomo 12 e é caracterizada por deficiência qualitativa ou quantitativa do fator de von Willebrand. A diversidade de mutações leva ao aparecimento das mais variadas manifestações clínicas possibilitando a divisão dos pacientes em vários tipos e subtipos clínicos. A coagulopatia se manifesta basicamente através da disfunção plaquetária associada à diminuição dos níveis séricos do fator VIII coagulante. O objetivo dessa revisão foi mostrar os cuidados relacionados aos pacientes portadores da doença de von Willebrand durante o período perioperatório.
CONTEÚDO: Foram definidas as características da doença de vonWillebrand quanto à fisiopatologia, à classificação, ao diagnóstico laboratorial, ao tratamento atual e aos cuidados com o manuseio do paciente no período perioperatório.
CONCLUSÕES:A doença de von Willebrand é o distúrbio hemorrágico hereditário mais comum, porém ela é subdiagnosticada pela complexidade da própria doença. A correta classificação do paciente, o uso apropriado da desmopressina e a transfusão do fator de von Willebrand são medidas fundamentais para a realização do procedimento anestésico bem-sucedido.

It is potentially analgesic and sedative, and could be used as adjuvant during general anesthesia, attenuating the blood pressure response to tracheal intubation and decreasing the need of anesthetics.
Literature recommends sugammadex at a dose of 16 mg.kg(-1) for patients with profound blockade. It was used in our patient with rapid and effective reversal of neuromuscular blockade allowing the patient to undergo another procedure to ensure the airway patency without clinical impairment of her general condition.
RESUMOBarbosa FT, Bernardo RC, Cunha RM, Pedrosa MSM -Anestesia Subaracnóidea para Cesariana em Paciente Portadora de Esclerose Múltipla. Relato de Caso.
JUSTIFICATIVA E OBJETIVOS:A esclerose múltipla é uma doença adquirida que se caracteriza por áreas desmielinizadas no encéfalo e na medula espinal. O quadro clínico depende das áre-as anatômicas acometidas. As principais causas de morte são infecção, falência respiratória e estado de mal epiléptico. Ocorre em pacientes geneticamente predispostos após contato com fatores ambientais, principalmente os vírus. O objetivo desse relato foi apresentar a técnica anestésica adotada em paciente com esclerose múltipla submetida à cesariana.
RELATO DO CASO:Paciente com 32 anos, 60 kg, portadora de esclerose múltipla, tratada com metilprednisolona, deu entrada no centro obstétrico para realização de cesariana. Após monitoração foi realizada anestesia subaracnóidea com bupivacaína a 0,5% hiperbárica (12,5 mg) associada à morfina (0,1 mg). O procedimento evoluiu sem intercorrências e a paciente recebeu alta hospitalar 48 horas após o parto sem piora dos sintomas preexistentes.
CONCLUSÕES:O presente caso sugere que a raquianestesia pode ser administrada em paciente portador de esclerose múltipla sem a ocorrência obrigatória de exacerbações agudas dos sintomas no período pós-operatório. Unitermos: DOENÇA: esclerose múltipla; TÉCNICAS ANESTÉSICAS, Regional: subaracnóidea.
