Objetivo: avaliar o desenvolvimento motor de recém nascidos prematuros extremos e comparar ao desenvolvimento motor dos recém-nascidos prematuros moderados, em uma UTI Neonatal. Métodos: estudo transversal onde foram avaliados 16 neonatos, cinco prematuros extremos e 11 prematuros moderados, classificados pelo Test of Infant Motor Performance. Resultados: Não houve diferença significativa no desempenho motor entre o grupo de prematuros extremos e moderados, entretanto, a prematuridade extrema reduziu 4,74 pontos no escore. Na correlação entre os fatores de risco e o desenvolvimento motor, o uso de ventilação mecânica não invasiva e o Apgar no 1º minuto foram associados estatisticamente com o desempenho motor atípico. As demais variáveis não apresentaram correlação significativa com o desenvolvimento motor. Conclusão: não houve associação entre a idade gestacional e o desenvolvimento motor de neonatos prematuros.

Delayed Motor Development and Infant Obesity as Risk Factors for Severe Deformational Plagiocephaly: A Matched Case–Control Study

The prevalence of deformational plagiocephaly (DP) has increased since the recommendation of positioning infants to their back during sleeping and is affected by various biological and environmental factors. This study aimed to investigate associations between DP and perinatal or infant characteristics, including obesity. This case–control study included 135 infants (81 males) aged 2–12 months who were diagnosed with DP using calculated cranial vault asymmetric index and cranial index and 135 age- and sex-matched controls. Motor development was evaluated using the Alberta Infant Motor Scale, and obesity was defined by body mass index. Univariate and multivariate logistic regression models were used to assess potential risk factors for DP and its severity. One hundred thirty-five infants with DP were divided into the following three subgroups according to severity indicated by the cranial vault asymmetry index: mild to moderate group ( n = 87, 64.4%), severe group ( n = 48, 35.6%), and a combined plagiocephaly and brachycephaly group ( n = 79, 58.5%). Independent risk factors significantly associated with development of DP were bottle-only feeding (adjusted odds ratio (aOR) = 4.65; 95% CI: 2.70–8.00), little tummy time when awake (aOR = 3.51, 95% CI: 1.71–7.21), delay of motor development (aOR = 2.85, 95% CI: 1.08–7.49), and obesity at diagnosis (aOR = 2.45, 95% CI: 1.02–5.90). Among these risk factors, delay of motor development (aOR = 4.91, 95% CI: 1.46–16.51) and obesity at diagnosis (aOR = 4.10, 95% CI: 1.42–11.90) were particularly related to severe DP. In conclusion, this study confirms that DP risk is positively associated with bottle-only feeding, infrequent tummy time, and delayed development of motor milestones. Notably, this study demonstrates infant obesity as a new risk factor for DP. Our findings suggest that obesity should be identified early and managed comprehensively in infants with DP.

IntroductionIn Scandinavia, 6% of infants are born preterm, before 37 gestational weeks. Instead of continuing in the in-utero environment, maturation needs to occur in a neonatal unit with support of vital functions, separated from the mother’s warmth, nutrition and other benefits. Preterm infants face health and neurodevelopment challenges that may also affect the family and society at large. There is evidence of benefit from immediate and continued skin-to-skin contact (SSC) for term and moderately preterm infants and their parents but there is a knowledge gap on its effect on unstable very preterm infants when initiated immediately after birth.Methods and analysisIn this ongoing randomised controlled trial from Stavanger, Norway and Stockholm, Sweden, we are studying 150 infants born at 28+0 to 32+6 gestational weeks, randomised to receive care immediately after birth in SSC with a parent or conventionally in an incubator. The primary outcome is cardiorespiratory stability according to the stability of the cardiorespiratory system in the preterm score. Secondary outcomes are autonomic stability, thermal control, infection control, SSC time, breastfeeding and growth, epigenetic profile, microbiome profile, infant behaviour, stress resilience, sleep integrity, cortical maturation, neurodevelopment, mother-infant attachment and attunement, and parent experience and mental health.Ethics and disseminationThe study has ethical approval from the Swedish Ethical Review Authority (2017/1135-31/3, 2019–03361) and the Norwegian Regional Ethical Committee (2015/889). The study is conducted according to good clinical practice and the Helsinki declaration. The results of the study will increase the knowledge about the mechanisms behind the effects of SSC for very preterm infants by dissemination to the scientific community through articles and at conferences, and to the society through parenting classes and magazines.Study statusRecruiting since April 2018.
