BackgroundDespite availability of effective cure, tuberculosis (TB) remains a leading cause of death in children. In many high-burden countries, childhood TB is underdiagnosed and underreported, and care is often accessed too late, resulting in adverse treatment outcomes. In this study, we examined the time to death and its associated factors among a cohort of children that commenced TB treatment in a large treatment centre in northern Nigeria.MethodsThis is a retrospective cohort study of children that started TB treatment between 2010 and 2014. We determined mortality rates per 100 person-months of treatment, as well as across treatment and calendar periods. We used Cox proportional hazards regression to determine adjusted hazard ratios (aHR) for factors associated with mortality.ResultsAmong 299 children with a median age 4 years and HIV prevalence of 33.4%; 85 (28.4%) died after 1,383 months of follow-up. Overall mortality rate was 6.1 per 100 person-months. Deaths occurred early during treatment and declined from 42.4 per 100 person-months in the 1st week of treatment to 2.2 per 100 person-months after at the 3rd month of treatment. Mortality was highest between October to December period (9.1 per 100 pm) and lowest between July and September (2.8 per 100 pm). Risk factors for mortality included previous TB treatment (aHR 2.04:95%CI;1.09–3.84); HIV infection (aHR 1.66:95%CI;1.02–2.71), having either extra-pulmonary disease (aHR 2.21:95%CI;1.26–3.89) or both pulmonary and extrapulmonary disease (aHR 3.03:95%CI;1.70–5.40).ConclusionsMortality was high and occurred early during treatment in this cohort, likely indicative of poor access to prompt TB diagnosis and treatment. A redoubling of efforts at improving universal health coverage are required to achieve the End TB Strategy target of zero deaths from TB.

Objetivos: Descrever as características dos casos de abandono do tratamento de tuberculose em pacientes que desenvolveram TBMR. Métodos: Estudo descritivo de abordagem quantitativa, desenvolvido em uma instituição terciária de referência para tratamento de TBMR localizada na cidade do Rio de Janeiro. Foi utilizado instrumento estruturado com informações de quarenta prontuários entre os meses de agosto a dezembro de 2016. A análise dos dados foi realizada através do software SPSS. Resultados: dos pacientes estudados, 35% abandonaram tratamento anterior de Tuberculose.Conclusão e implicação para a prática: é necessária a identificação precoce de pacientes que apresentam maior risco para abandono do tratamento, bem como a criação de um modelo de assistência voltado ao perfil dos usuários que abandonam o tratamento, com uma prática participativa, com grupos educativos que promovam ações preventivas, identificando, estudando estratégias para a superação de barreiras ligadas à realidade, de vida individual e coletiva.

RESUMOObjetivo: Descrever as características dos casos de abandono do tratamento de tuberculose em pacientes que desenvolveram TBMR. Método: Estudo de abordagem quantitativa, desenvolvido em um hospital terciário de referência para tratamento de TBMR localizada no municipio do Rio de Janeiro. Foi utilizado instrumento estruturado com informações de oitenta prontuários entre os meses de fevereiro a dezembro de 2017. A análise dos dados foi realizada através do software SPSS. Resultados: Foi possível verificar que dos 80 pacientes estudados: 35% abandonaram tratamento anterior de Tuberculose, a média de idade foi de 43,6 anos; 67,5% dos pacientes eram do sexo masculino e 42,5% pardos. Conclusão:As evidências desse estudo apontam que é necessária a identificação precoce de pacientes que apresentam maior risco para abandono do tratamento, bem como a criação de um modelo de assistência voltado ao perfil dos usuários que abandonam o tratamento, com uma prática participativa dos profissionais de saúde, com grupos educativos que promovam ações preventivas, identificando, estudando estratégias para a superação de barreiras ligadas à realidade, de vida individual e coletiva. Method: A quantitative approach, developed in a tertiary reference hospital for the treatment of TBMR located in Rio de Janeiro. A structured instrument with information from eighty medical records was used between February and December 2017. Data analysis was performed using SPSS software. Results: It was possible to verify that about the 80 patients studied, 35% abandoned previous treatment of Tuberculosis, the mean age was 43.6 years, 67.5% of the patients were male, 42.5% were mixed race. Conclusion: Evidence from this study indicates that it is necessary to identify patients at major risk for treatment abandonment, as well as to create a model of assistance aimed at the profile of users who abandon treatment, with a participative practice of the professionals of health, with educational groups that promote preventive actions, identifying, studying strategies for overcoming barriers linked to reality, individual and collective life.
