Імовірність потрапляння неодимових магнітів у шлунково-кишковий тракт (ШКТ) дітей дуже висока, однак досі немає єдиної тактики з їх вилучення: одні автори рекомендують вичікувати, інші наполягають на негайному ендоскопічному видаленні або оперативному втручанні. Мета: удосконалення тактики лікування дітей із магнітами ШКТ. Матеріали і методи. Під спостереженням було 7 дітей із магнітами ШКТ. Усім дітям проведено повне загальноклінічне та інструментальне обстеження (УЗД, Rg ОЧП). Результати. Всі випадки є екстреними. При обстеженні локалізація магнітів у шлунку спостерігалась у 3 (42,9%) дітей: 1-самостійне відходження, 1-вилучення за допомогою ФЕГДС, 1-гастротомія. У 4 (57,1%) дітей магніти були у кишковому тракті: проведено лапаротомії з приводу перитоніту та кишкової непрохідності. Одужали 6 (85,7%) дітей. За результатами аналізу клінічних випадків розроблено алгоритм дій за підозри на магніти ШКТ. Висновки. За підозри на потрапляння у ШКТ неодимових магнітів не можна очікувати на вихід стороннього тіла природним шляхом. Обов'язково потрібно ретельно зібрати анамнез, провести Rg ОЧП, УЗД. Подальша тактика залежить від кількості магнітів та терміну їх перебування у ШКТ: чим більше магнітів, тим швидше потрібно виконати лапаротомію. Оцінити можливість використання боратного скломатеріалу під час лікування трофічних ран у дітей з мієлодисплазією. Матеріали і методи. Представлено дані щодо 3 дітей, які у періоді новонародженості були оперовані з приводу спинномозкових гриж. Усі діти через мієлодисплазію мали порушену трофіку нижніх кінцівок, що обумовило тяжкий перебіг загоювання ран або пролежнів. Біоактивне боратне скло «Mirragen» виробництва США для лікування трофічних ран у цих дітей запропонував медичний центр «Медиво». Результати. Застосування біоактивного боратного скла під час лікування трофічних ран у дітей з мієлодисплазією було ефективне. Глибокі рани протягом 8 тижнів повністю загоїлись без ускладнень, а поверхнева рана-пролежень -протягом 5 тижнів. Рани поступово епітелізувалися як з глибини, так із периферії зі швидкістю до 12-15% площі на тиждень. Негативної загальної або місцевої реакції на боратне скло у пацієнтів не спостерігали. Висновки. Тимчасовий біоматеріальний каркас із боратного скла забезпечує необхідну форму для утворення нових тканин і сприяє швидкому загоєнню ран. Застосування біоактивного боратного скла є ощадливим та ефективним методом лікування трофічних ран у дітей з мієлодисплазією. Ключові слова: біоактивне боратне скло; рани, що тяжко загоюються; діти. AbstractObjective. To estimate the possibility of application of a borate-containing glass material while treating trophic wounds in children, suffering myelodysplasia. Маterials and methods. The data, concerning 3 children, who, while their newborn time were operated on for a spinal cord hernias, are presented. All the children have had myelodysplastic disordered trophic wound of the lower extremities, causing severe course of the wounds or the bedsores healing. Bioactive borate-containing glass «Mirragen», manufactured in USA, was proposed by medical centre «Меdyvo» for treatment of trophic wounds in children. Results. Application of bioactive borate-containing glass while treating trophic wounds in children, suffering myelodysplasia, was effective. Deep wounds during 8 weeks have had healed completely without complications, and a superficial wound-bedsore -during 5 weeks. The wounds have epithelized steadily, starting together from the bottom and periphery with velocity up to 12-15% of their square weekly. Negative general or local reaction towards a borate-containing glass in the patients was absent. Conclusion. Тemporary biomaterial, structured by a carcass, made of a borate-containing glass, guarantees a necessary form establishment, needed for creation of a new tissues and promotion of rapid healing of the wounds. Application of a bioactive boratecontaining glass constitutes a safe and efficient method of treatment for trophic wounds in children, suffering myelodysplasia.
