Імовірність потрапляння неодимових магнітів у шлунково-кишковий тракт (ШКТ) дітей дуже висока, однак досі немає єдиної тактики з їх вилучення: одні автори рекомендують вичікувати, інші наполягають на негайному ендоскопічному видаленні або оперативному втручанні. Мета: удосконалення тактики лікування дітей із магнітами ШКТ. Матеріали і методи. Під спостереженням було 7 дітей із магнітами ШКТ. Усім дітям проведено повне загальноклінічне та інструментальне обстеження (УЗД, Rg ОЧП). Результати. Всі випадки є екстреними. При обстеженні локалізація магнітів у шлунку спостерігалась у 3 (42,9%) дітей: 1-самостійне відходження, 1-вилучення за допомогою ФЕГДС, 1-гастротомія. У 4 (57,1%) дітей магніти були у кишковому тракті: проведено лапаротомії з приводу перитоніту та кишкової непрохідності. Одужали 6 (85,7%) дітей. За результатами аналізу клінічних випадків розроблено алгоритм дій за підозри на магніти ШКТ. Висновки. За підозри на потрапляння у ШКТ неодимових магнітів не можна очікувати на вихід стороннього тіла природним шляхом. Обов'язково потрібно ретельно зібрати анамнез, провести Rg ОЧП, УЗД. Подальша тактика залежить від кількості магнітів та терміну їх перебування у ШКТ: чим більше магнітів, тим швидше потрібно виконати лапаротомію. Pulmonary artery thromboembolia (PATE) is not a clinical entity as such, but a complication of different diseases and conditions leading to deep thrombosis in the low tension circulation system, right cardiac chambers or causing local thrombosis in the pulmonary artery system. PATE is characterized by complex pathogenesis of respiratory failure of varying severity, which makes it difficult to choose a respiratory support technique.Purpose of the overview: to show advantages of different respiratory support techniques and prospects of high-flux oxygen therapy with regard to PATE pathophysiology.82 sources were selected based on the principle of combining clinical and experimental data from papers published over the recent 5 years and earlier that are still relevant for medical practice.The overview presents the structure of main causes and prevalence of PATE and considers thrombogenesis stages and predominant manifestations of respiratory failure during PATE occurring due to inconsistency between pulmonary ventilation and perfusion. Review discusses five methods of respiratory therapy in PATE patients: low-flux and high-flux oxygen therapy, non-invasive and invasive artificial lung ventilation, extracorporeal blood oxygenation. Finally, the paper shows the efficacy and limitations of these methods.Conclusion. High-flux oxygen therapy seems to be the most effective and promising technique in PATE patients thanks to absence of adverse cardiohemodynamic consequences, subjective comfort for patients, and relation to minimal risks of secondary infectious complications.Nevertheless, the clinical experience accumulated is insufficient to make an absolute choice of one particular technique for respiratory support during PATE. It is necessary to continue investigating the clinical efficacy of high-flux oxygen therapy in the specific population of patients who experienced PATE.
A total of 93 children with pleural complications of destructive pneumonia were randomised, and all the patients get studymethod of dynamic thoracoscopy. Evaluation of changes in lung and pleura were made. Basing to this changes-the stages of destructive process were developed. On the base of the stages patogenetical treatment was suggested. In the treatment group (n=56) method of fractional washing of pleural cavity, and the method of local antibioticotherapy was used. No complications resulted from the thoracoscopies and there was no need for further surgical intervention in any of these patients. We conclude that dynamic thoracoscopy allows for minimally invasive, yet effective treatment pleural complications of destructive pneumonia. Thoracoscopic visualization of the pleural cavity permits efficient debridement, thorough adhesiolysis, and optimal placement of drainage tubes.

Інтенсивна консервативна терапія, яка включає повне парентеральне харчування та медикаментозне лікування, є важливим компонентом післяопераційного лікування дітей із СКК. Ця патологія вимагає мультидисциплінарного підходу, а хірургічні втручання допомогають досягти автономності кишечника. Омфалоцеле, наряду с гастрошизисом, является одним из частых врожденных пороков передней брюшной стенки. Обычно омфалоцеле характеризуют по размеру, локализации и состоянию мешка, выделяя небольшие, средние и гигантские; эпигастральные, центральные и гипогастральные; с неповрежденным или разорванным мешком. Выбор метода лечения новорожденного с гигантским омфалоцеле является предметом дискуссий среди детских хирургов. Цель исследования – представить различные методы консервативного и хирургического лечения гигантского омфалоцеле.Основной задачей лечения новорожденного с омфалоцеле является вправление органов брюшной полости и закрытие дефекта передней брюшной стенки без существенного повышения внутрибрюшного давления.Нехирургические методы лечения новорожденных с гигантским омфалоцеле направлены на вправление эвентрированных органов в брюшную полость с последующей отсроченной хирургической пластикой дефекта передней брюшной стенки. Нехирургические методы лечения включают использование метода Schuster, эластического бинтования, аппаратов для создания отрицательного давления, топического нанесения препаратов для эскарификации мешка и другие. Эти методы могут применяться в отдельности или же в сочетании друг с другом.При хирургическом лечении гигантского омфалоцеле выделяют ранние и отсроченные вмешательства, последние можно разделить еще на две группы – выполняющиеся после поэтапного вправления органов и после полной эпителизации грыжевого мешка. Первичная пластика при гигантском омфалоцеле возможна только в единичных случаях. Методы первичной пластики включают метод Gross, метод разделения компонентов брюшной стенки, а также использование различных сеток или биопротезов.Отсроченные вмешательства выполняются для закрытия фасциального и кожного дефектов, после полного вправления эвентрированных органов или лечения вентральной грыжи, образовавшейся после эпителизации мешка. Omphalocele, along with gastroschisis, is one of the most frequent congenital malformations of the abdominal wall. Traditionally, omphalocele is characterized by size, localization, and sac condition. Therefore, they may be small, minor or giant; epigastric, central or hypogastric; intact or ruptured. The choice of the method of treatment of newborns with giant omphalocele is still under debate among pediatric surgeons.The purpose of this study was to present various methods of non-operative and surgical treatment of giant omphalocele.The main goal of the management of newborns with omphalocele is the reduction of abdominal contents and closure of the fascial and skin defects avoiding intolerable increase of intraabdominal pressure.The non-operative approaches to the management of newborns with giant omphalocele are directed at the gradual reduction of herniated viscera with the delayed closure of the abdominal wall defect. Non-operative approaches include Schuster’ method, elastic banding, devices for negative pressure therapy, topical solutions for epithelization of the sac and so on. These methods can be applied separately or combined.Early and delayed surgical closures are distinguished in the surgical treatment of giant omphalocele. The delayed closure can be divided in two groups: after gradual reduction of the viscera and after epithelization of the sac.The primary repair of giant omphalocele is appropriate only in selected cases. Skin flaps (Gross method), the component separation technique, and applying various synthetic nets or bioprosthetic material are used for the primary repair.The delayed repair is directed to the closure of the fascial and skin defects after reducing the herniated viscera or to the treatment of ventral hernia that developed after the sac epithelization.
