Ленина, 3 РезюмеРеспираторная и сердечно сосудистая системы тесно взаимосвязаны, при этом в случае хронической обструктивной болезни легких (ХОБЛ) на их статус может оказать влияние терапия бронходилататорами. На примере 2 клинических наблюдений рассматривается воз можность перехода от монотерапии индакатеролом к комбинированной терапии индакатеролом и гликопиррония бромидом с анали зом динамики изменений соответствующих респираторных и кардиоваскулярных показателей у пациентов с ХОБЛ тяжелой степени. Продемонстрирована тенденция к уменьшению бронхообструкции, улучшению субъективных ощущений, сократительной способнос ти левого и правого желудочков и вариабельности ритма сердца. SummaryThe aim of this article was to investigate respiratory and cardiovascular effects of combined therapy with indacaterol and glycopyrronium bromide in patients with severe chronic obstructive pulmonary disease. Methods. Two clinical cases are reported in the article. Both patients were treatment with long acting beta 2 agonist indacaterol for 3 months followed by 3 month treatment with combined therapy with indacaterol and glycopyrro nium bromide. We assessed arterial stiffness, pulse wave velocity rate, the heart rhythm variability, used spirometry and echocardiography, CAT and MMRC scales. Results. Combined inhalation therapy resulted in significant increase in FEV1 and FVC, reduction in dyspnea, improvement in arte rial oxygen saturation. Arterial stiffness was not changed, but the heart rhythm autonomic regulation improved in both patients. A tendency to increase in the left heart ejection fraction was also found. Conclusion. Our data are consistent with results of other clinical trials of indacaterol and glycopyrronium bromide in patients with chronic obstructive pulmonary disease.
Хроническая обструктивная болезнь легких (ХОБЛ) представляет собой поддающееся лечению и профи лактике заболевание, которое характеризуется стой ким снижением объема воздушного потока и сопро вождается патологической воспалительной реакцией в дыхательных путях и легких в ответ на воздей ствие газов и частиц [1]. В качестве терапии первой линии при ХОБЛ легкой или средней степени тя жести рекомендуется применение бронходилата торов. В настоящее время существуют бронходила таторы различного механизма действия и типов -β адреномиметики короткого действия, М холино литики короткого действия, длительно действующие β адреномиметики (ДДБА) и М холиномиметики (ДДХМ) [1]. В настоящее время предпочтение отда ется препаратам длительного и ультрадлительного действия.Молекула индакатерола создана из модифициро ванной гидрофильной головной группы молекулы формотерола, что обеспечивает его высокую внут реннюю активность, и липофильного хвоста -более ригидного, компактного и короткого по сравнению с салметеролом [2]. Это позволяет отнести данный препарат к β агонистам ультрадлительного действия. Рекомендуемая доза индакатерола для пациентов с ХОБЛ составляет 150 или 300 мкг 1 раз в сутки, для доставки препарата в дыхательные пути использует ся капсульный дозированный порошковый ингаля тор Бризхалер.ХОБЛ различной степени тяжести в более чем в 50 % случаев сопутствуют взаимоутяжеляющие кардиоваскулярные заболевания, такие как ишеми ческая болезнь сердца (ИБС), артериальная гипер тензия (АГ), аритмии и другая патология [3, 4], раз витию которых способствуют средний и пожилой возраст пациентов, курение, низкий комплаенс и т. п. Показано, что многие препараты, которые ис пользуются в лечении ХОБЛ, оказывают негативное действие на сердечно сосудистую систему, напри мер, при использовании β агонистов и метилксан РезюмеРеспираторная и сердечно сосудистая системы тесно взаимосвязаны, при этом терапия бронходилататорами при хронической обструк тивной болезни легких (ХОБЛ) может оказывать влияние и на состояние сердечно сосудистой системы. Цель. Изучение влияния ингаля ционного β агониста индакатерола на состояние сердечно сосудистой системы у больных ХОБЛ, в т. ч. в зависимости от приверженности лечению. Материалы и методы. В проспективном нерандомизированном исследовании у пациентов (n = 25) с ХОБЛ среднетяжелой сте пени, принимавших индакатерол, в течение 3 мес. до и после терапии проведено эхокардиографическое обследование, определены ар териальная жесткость, скорость пульсовой волны, вариабельность ритма сердца и комплаентность терапии. Результаты. Через 3 мес. отмечена тенденция к уменьшению размеров полостей сердца, снижению давления в легочной артерии, улучшению сократительной способности сердца и улучшению вегетативного статуса. При этом достоверных изменений функции внешнего дыхания не отмечено, а наиболее выраженные изменения наблюдались у 18 комплаентных пациентов. Заключение.
