The article discusses modern principles of diagnostics and treatment of acute streptococcal pharyngitis and tonsillitis in different age groups. It gives recommendations on using an express test for quick and quality diagnostics of infections caused by group A β-hemolytic streptococcus (GABHS) in routine practice. The introduction of recommendations on diagnostic and therapeutic tactics in adult patients into this article allows to resolve issues of managing family cases of GABHS-infection. Adequate conduct of GABHS express tests and rational selection of therapeutic measures at acute pharyngitis and tonsillitis reduces the risk of prescribing ungrounded antimicrobial therapy.
Синдром периодической лихорадки с дефицитом мевалонаткиназы относится к группе наследственных форм периодической лихорадки, отличается ранним началом и относительно благоприятным течением [1][2][3]. Другое, более распространенное его название -синдром гипериммуноглобулинемии D -отражает тот факт, что у больных обычно обнаруживается повышен-ный уровень иммуноглобулина D. Следует заметить, что у некоторых больных с генетически подтвержденным синдромом уровень иммуноглобулина D не повышен. Это прежде всего касается детей в возрасте до 3 лет [4]. Синдром «голландской лихорадки» впервые описал Ван Дер Меер в 1984 г., который обозначил его как сочета-ние фебрильных приступов и высокого уровня иммуно-глобулина D в плазме крови [2]. До сих пор связь между синдромом периодической лихорадки и повышением иммуноглобулина D неясна, но, по-видимому, иммуно-глобулин D не участвует непосредственно в патогенезе болезни. В мировой литературе термины «синдром пери-одической лихорадки с дефицитом мевалонаткиназы» и «синдром гипериммуноглобулинемии D» используются как синонимы. КЛИНИЧЕСКИЕ ПРОЯВЛЕНИЯ И ДИАГНОСТИКА СИНДРОМАСиндром периодической лихорадки с дефицитом мева-лонаткиназы отличается ранним началом, в среднем пер-вые 6 мес жизни. Эпизоды высокой лихорадки с ознобами продолжаются 3-7 дней и возникают с периодичностью 1 раз в 2-6 нед [5]. Приступы могут иметь более или менее выраженную периодичность. Факторами, провоцирующи-ми обострения, выступают физическая или эмоциональ-ная нагрузка, либо иммунизация. Фебрильные приступы могут возникать и без видимых причин [6].Чаще всего лихорадка сопровождается одно-или двусторонней, иногда болезненной, шейной лимфоаде-нопатией (98% пациентов), пятнисто-папулезной сыпью (85%), болями в животе (85%), сплено-и гепатомегалией (50%), артралгиями (80%). Очень часто во время лихо-радки возникают рвота и диарея, изменения слизистых оболочек (эрозии слизистой оболочки ротовой полости и половых органов, как при болезни Бехчета), миалгии, боли в горле [3,6].Между лихорадочными приступами самочувствие ребенка удовлетворительное.
