В первой половине прошлого года в журнале «Annals of the Rheumatic Diseases» были опубликованы новые между-народные рекомендации по диагностике, лечению и про-филактике подагры [1].В создании этих рекомендаций приняли участие око-ло 500 практикующих ревматологов из 14 стран Европы и Америки, перед которыми международным ревматологи-ческим объединением «Тhe 3e initiative» (Evidence -дока-зательство, Expertise -оценка, Exchange -обмен) была поставлена задача адаптировать существующие рекомен-дации для применения в клинической практике, тща-тельно изучив литературу последних лет, посвященную проблеме подагры, и максимально кратко и понятно ос-ветить для практикующих врачей десять следующих воп-росов:1. На основании каких критериев (клинических, лабо-раторных, инструментальных) может быть установлен диаг-ноз подагры?2. Следует ли проводить обязательный скрининг паци-ентов с гиперурикемией или установленным диагнозом по-дагры на наличие сопутствующих заболеваний и факторов кардиоваскулярного риска? 3. Какова роль колхицина, глюкокортикоидов (ГК), не-стероидных противовоспалительных препаратов (НПВП) и других средств в лечении острой подагры?4. Влияет ли модификация образа жизни (диета, огра-ничение потребления алкоголя, снижение массы тела, отказ от курения и занятия лечебной физкультурой) на течение и предупреждение подагры?5. Каковы эффективность и безопасность применения уратснижающих препаратов (аллопуринола, фебуксостата, бензбромарона, пробенецида) при подагре? Какую после-довательность или комбинацию таких препаратов следует использовать?6. Как предотвратить острый приступ подагры при про-ведении уратснижающей терапии и когда необходимо начи-нать такую терапию?7. Влияет ли сопутствующая патология (метаболический синдром, кардиоваскулярные, желудочно-кишечные заболе-вания, почечная недостаточность) на выбор уратснижающих препаратов (колхицина, аллопуринола и др.) при хрониче-ской подагре, остром приступе артрита и его профилактике?8. Какова цель лечения подагры и как следует наблю-дать таких пациентов?
ЭпидемиологияУстановлено, что риск развития токсичности амио дарона возрастает по мере увеличения его концент рации в плазме крови. Так, при ежедневном приеме Резюме В обзоре представлены литературные данные об одном из самых опасных побочных эффектов антиаритмического препарата III класса амиодарона -поражении легких, т. н. амиодарон индуцированной легочной токсичности (АИЛТ). Проанализированы данные о рас пространенности этого осложнения, факторов, предрасполагающих к его развитию, взаимосвязь дозировок, длительности приема амио дарона и вероятности развития АИЛТ. Освещены вопросы клинической фармакологии и наиболее распространенных побочных эффек тов амиодарона. Описаны патофизиологические, патоморфологические особенности, клинические проявления и осложнения АИЛТ, включая диффузный пневмонит, бронхиолит, дыхательную недостаточность и острый респираторный дистресс синдром. С целью ди агностики АИЛТ отмечены характерные результаты лабораторных, инструментальных методов исследования, в частности оценка функ ции внешнего дыхания, рентгенография органов грудной клетки, компьютерная томография высокого разрешения и биопсия легких. Лечебные приемы при ведении пациентов с АИЛТ включают снижение дозировки, полное прекращение приема препарата и назначе ние глюкокортикостероидов. В профилактических целях для снижения риска развития АИЛТ рекомендована изначальная спирометри ческая оценка функции легких, рентгенографический контроль и информирование пациентов о возможных проявлениях этого тяжело го легочного осложнения при лечении амиодароном. SummaryThis is a review of current literature on lung injury which is one of the most hazardous adverse effects of an antiarrhythmic agent amiodarone referred to as amiodarone induced pulmonary toxicity (AIPT). Finding on AIPT prevalence, risk factors, dose dependence, therapy duration and probabil ity of AIPT occurrence have been analyzed. Issues of clinical pharmacology and the most common adverse effects of amiodarone have been dis cussed. Pathophysiological and pathological features and clinical manifestations of AIPT including diffuse pneumonitis, bronchiolitis, respiratory failure and acute respiratory distress syndrome have been described. Diagnosis of AIPT is based on routine methods such as lung function tests, chest X ray, high resolution computed tomography, and lung tissue biopsy. Management approaches include dose reduction or drug withdrawal and administration of systemic steroids. Preventive measures include baseline spirometry, regular X ray examination and information about symptoms of this complication provided to the patients.
