Increasing life expectancy is the goal of social programs in the Russian Federation as a reflection of the success of public health. Globally, there is a trend for aging of the population, contributing to an increased risk for lung cancer developing which is primarily a disease of the elderly. Chronologic age or performance scores alone are not accurate predictors of patients’ capacity for tolerating aggressive cancer therapies. Use of a comprehensive geriatric assessment to determine treatment strategy is able to reduce toxicities and treatment failures. Safe elderly patients are often able to tolerate surgical resection, radiation, and/or chemotherapy appropriate for their tumor stage, with outcomes similar to those of younger patients, albeit with higher rates of treatment-related toxicity.
The discovery of immune checkpoint inhibition has revolutionized the treatment of many solid malignancies, including non-small cell lung cancer (NSCLC). Immune checkpoint inhibitors (ICI) can restore the antitumor immune response by blocking the inhibition of T-cell activation. Anti-programmed death-ligand 1 (PD-L1) is currently the main biomarker of the effectiveness of anti-PD-1 / PD-L1 blockade in the treatment of NSCLC without driver mutations. High tumor mutational burden suggests an increased neoantigens load and has been associated with the effectiveness of ICI therapy. Microsatellite instability, a biomarker approved for immunotherapy across solid tumors, but it is uncommon in NSCLC. Primary resistance to ICIsis characteristic of NSCLC with driver mutations, acquired is associated with immunoediting resulting in the depletion of potentially immunogenic neoantigens. The review discusses recent advances and future directions for predicting the results of immunotherapy in patients with NSCLC.
Несмотря на некоторое снижение заболеваемости, доля рака легкого (РЛ) в структуре онкологической патологии населения в России по-прежнему занима-ет ведущее место, а каждый 3-й мужчина, умерший от рака в нашей стране -это больной РЛ [1].Результаты хирургического лечения, остающего-ся основным методом при немелкоклеточном РЛ (НМРЛ), вряд ли можно считать удовлетворитель-ными даже при I стадии опухолевого процесса [2,3].S. Garbor et al. [4] было продемонстрировано, что опухолевая инвазия в кровеносные сосуды является важным прогностическим фактором у пациентов c T13N0M0 стадиями НМРЛ.В другом исследовании изучено влияние опухо-левой сосудистой инвазии у пациентов с I или II ста-дией НМРЛ, которая была выявлена только у 14,4 % больных, у которых отмечена тенденция к ухудше-нию бессобытийной выживаемости [5]. Японскими учеными также показано, что общая выживаемость больных с IA стадией РЛ с опухолевой сосудистой инвазией была аналогичной у пациентов с IB стади-ей НМРЛ [6]. Установлена зависимость от микросо-судистой инвазии [7].В этой связи представляет интерес дальнейшее изучение опухолевой сосудистой инвазии, которая, наравне со стадией, может являться значимым про-гностическим фактором для радикально проопери-рованных пациентов с НМРЛ.Целью явилось исследование в образцах удален-ной опухоли инвазии сосудов микроциркуляторного русла при НМРЛ путем иммуногистохимического (ИГХ) определения экспрессии маркеров: CD34 + , мышечного актина, цитокератина и установление их прогностического значения.
