Российский кардиологический журнал № 5 (103) | 2013Согласно данным многих исследований, в основе развития атеросклероза лежит последовательное взаи-модействие многих этиопатогенетических факторов, ведущее, в конечном счете, к образованию атеросклеро-тической бляшки [1][2][3].Начальная стадия атеросклероза характеризуется появлением в интиме артерий пятен и полосок, содер-жащих липиды. Липидные пятна появляются в арте-риях с раннего детства. В возрасте 10 лет липидные пятна занимают около 10% интимы аорты, а к 25 годам -от 30% до 50%. В коронарных артериях липои-доз встречается с 10-15 лет, в артериях мозга -в конце третьего десятилетия жизни (к 35-45 годам) [4].Липидные пятна представляют собой участки жел-товатого цвета небольших размеров (до 1,0-1,5 мм) в интиме аорты или крупных артерий. Липидные пятна состоят, главным образом, из пенистых клеток, содер-жащих большое количество липидов, особенно холе-стерина (ХС), и Т-лимфоцитов (Т-ЛФ). Также в липид-ных пятнах присутствуют макрофаги (МФ) и гладко-мышечные клетки (ГМК). Для липидных пятен характерно преимущественно внутриклеточное нако-пление эфиров ХС. Вокруг липидных пятен наблюда-ется образование соединительной ткани (рис. 1). Со временем липидные пятна увеличиваются в разме-рах, сливаются друг с другом и образуют так назы- ваемые липидные полоски, возвышающиеся над поверхностью эндотелия. Они также состоят из МФ, Т-ЛФ, ГМК и пенистых клеток, нагруженных липи-дами и ХС. На этой стадии развития атеросклероза ХС расположен преимущественно внутриклеточно и лишь малое его количество находится вне клеток. В основе начального этапа развития атеросклеротического очага лежит повреждение эндотелиоцитов и развитие их дис-функции, сопровождающейся повышением проницае-мости эндотелия [5][6][7]. По мере прогрессирования патологического про-цесса в участках отложения липидов разрастается сое-динительная ткань, что ведет к образованию фиброз-ных бляшек, в центре которых формируется липидное ядро. Этому способствует увеличение количества липи-дов, высвобождающихся в результате апоптоза ГМК, МФ и пенистых клеток, перегруженных ХС. Экстра-целлюлярно расположенные липиды пропитывают интиму, образуя липидное ядро, которое представляет собой скопление атероматозных масс (липидно-белко-вого детрита). Вокруг липидного ядра возникает зона соединительной ткани, вначале богатой клеточными элементами (МФ, пенистыми клетками, ГМК, Т-ЛФ), коллагеном и эластином (рис. 2). Цель. Прогнозирование отдаленных (годовых) неблагоприятных и благопри-ятных исходов острого коронарного синдрома со стойким подъемом сегмента ST (ОКСпST) для оптимизации реабилитации, вторичной профилактики и пер-сонифицированного подхода к лечению. Материал и методы. В исследование были включены 145 больных ОКСпST (106 мужчин и 39 женщин), госпитализированных в ГКБ № 1 г. Новосибирска в течение 2010г. Средний возраст больных составил 59,1±6,1 лет. Диагноз ОКС устанавливался по совокупности критериев, разработанных Европейским обще-ством кардиологов и Американской коллегией кардиологов (2000г). Всем паци-ентам было проведено клинико-инструментальное исследование по следующей программе: клинический осмотр, электрокардиография, эхокардиография, хол-теровское мониторирование, исследование воспалительных цитокинов и моле-кулярно-генетические исследования. Методами факторного и корреляционного анализа в работе было определено влияние каждого из исследованных показате-лей на вероятность неблагоприятного годового прогноза. Результаты. Результатом исследования явилась инновационная методика многофакторного прогнозирования отдаленных (годовых) неблагоприятных и благоприятных исходов острого коронарного синдрома со стойким подъе-мом сегмента ST. Модель включает в себя определение наличия или отсут-ствия у пациента сахарного диабета (СД), величины фракции выброса левого желудочка (ФВ ЛЖ), концентрации высокочувствительного С-реактивного протеина в сыворотке крови (вчСРП), генотипа по полиморфному варианту rs1376251 гена TAS2R50. Чувствительность предлагаемого способа по про-гнозированию неблагоприятных исходов составила 82%, благоприятных исхо-дов -80%. Заключение. Предложенная модель разработана на основе исследования показателей у российских пациентов и учитывает активность атеросклероти-ческого воспалительного процесса и генотип пациента. Формула расчета вероятности неблагоприятного исхода показывает, с каким весовым множите-лем тот или иной фактор влияет на вероятность неблагоприятного исхода. Модель проста в использовании и позволяет индивидуализировать вторичную профилактику у данной категории пациентов.
Background.A functionally significant TP53Arg72Pro polymorphism can contribute to the development of gastric cancer (GC).The aim:to study the associations of genotypes and alleles of the TP53Arg72Pro 4 polymorphism with GC and biomarkers of gastric ucosal atrophy in population-based prospective and case-control clinical trials among the population of Siberia.Material and methods.As a part of the epidemiological study, data of the international multicenter HAPIEE project for 2003–05, based on a population sample of residents of Novosibirsk city (serum and DNA samples) and data of the population-based registry of GC (2012) were compared. Gastric cancer patients were matched by age and sex to HAPIEE population controls. A total of 156 serum samples (GC – 52, control – 104) and 146 DNA samples (GC – 50, control – 96) were available for prospective analysis. DNA samples from 80 gastric cancer patients (45 men and 35 women, mean age 61.0 ± 13.4 years) and from 87 age-and sex-matched controls were analyzed. DNA samples from venous blood were genotyped according to standard methods. Serum samples were tested using diagnostic kit for enzyme-linked immunosorbent assays to determine the levels of pepsinogen I (PGI), PGII, PGI/PGII ratio, gastrin-17 and IgG antibodies to H. pylori.Results.No differences in genotype and allele frequencies of the TP53 gene between the case group and the control group were found. A decreased frequency of the Pro allele in female gastric cancer patients compared with controls indicated that the Pro allele is protective against the development of gastric cancer, but this effect was not observed in male patients. No associations of TP53 genotypes with the risk of diffuse or intestinal gastric cancer, as well as with the age and sex of patients were found. A high frequency of genotypes with the Pro allele in patients with stage III–IV gastric cancer indicated the relationship between Arg/Pro TR53 and tumor progression, in particular, the contribution of the minor Pro allele to the unfavorable prognosis. A prospective study showed high risk of reducing the level of pepsinogen for assessing predisposition to gastric cancer.Conclusion.Two case-control studies (population and clinical) conducted in the Western Siberia found no relationship between the TP53Arg72Pro polymorphism and the risk of gastric cancer. However, the TP53 genotype with a rare Pro allele was associated with atrophic gastritis and severity of gastric cancer.
