Aim. To present the comparative clinical characteristics of chronic hepatitis C infection and to estimate the effect of antiviral treatment in Caucasian and Mongoloid patients, considering interleukin-28B gene polymorphism in study populations.Methods. Population and genetic study including 1520 healthy subjects and 267 patients with chronic viral hepatitis of Caucasian and Mongoloid race analyzed interleukin-28B gene polymorphism. 433 patients with chronic viral hepatitis C were followed up. Combined antiviral treatment by pegylated interferon alfa-2a and ribavirin was administered for 48 weeks. The rates of early and sustained virologic response were the endpoints for assessing antiviral treatment effect. 82 Caucasian patients and 42 Mongoloid patients have completed the full course of antiviral treatment and 24 weeks of follow-up.Results. The following clinical features of chronic hepatitis C were revealed in Mongoloids: more extensive liver fibrosis, older age, longer infection duration and higher total cholesterol levels (due to triglycerides). These factors negate the expected better effect of antiviral therapy effect, associated with higher prevalence of interleukin-28B favorable genotypes in Mongolian population. The presence of the C-allele of rs12979860 gene and T-allele of rs8099917 gene reaches 89.6 and 91.7% respectively in Mongolian population, which is comparable with those in China, Japan and Korea, and is typical for the whole of Asia. Combined treatment with pegylated interferon alfa-2a and ribavirin in patients with genotype 1 of hepatitis C virus led to sustained virologic response in 78.5% of Mongoloid and in 56.1% of Caucasian patients; groups were totally comparable.Conclusion. Mongoloid patients with chronic hepatitis C reached sustained virologic response to antiviral treatment with pegylated interferon and ribavirin significantly more frequently compared to Caucasians, in cases of no aggravating factors and minimal liver fibrosis.
ЭНДОКРИНОЛОГИЯ ВВЕДЕНИЕВ структуре метаболических заболеваний ожирение занимает первые позиции. По оценкам ВОЗ, более 1,9 млрд человек в мире имеют избыточный вес, из них свыше 600 млн страдают ожирением [1]. Изучение патоге-нетических особенностей при ожирении является одним из приоритетных направлений в современной медицине. В настоящее время жировая ткань рассматривается как активный эндокринный орган, производящий большое количество гормонов (адипокинов) [2,3]. Гормоны жиро-вой ткани оказывают влияние практически на все системы организма человека, регулируя чувствительность к инсули-ну и энергетический обмен, нейроэндокринные и иммун-ные процессы, активируют свертываемость крови и вос-палительные реакции [3,4]. В свою очередь, избыточное накопление жировой ткани неизменно приводит к изме-нению метаболических процессов.Цель работы: провести анализ метаболизма гормонов жировой ткани у пациентов с ожирением.

Цель исследования -установить долю пациентов с критическими значениями (КЗ) от общего числа пациентов и долю критических результатов для разных медицинских организаций, частоту КЗ по аналитам и местоположение пациентов с КЗ; попытаться оценить возможности их интерпретации в контексте особенностей внелабораторной части преаналитического этапа в каждой лаборатории. Материал и методы. В пилотном исследовании участвовало 6 медицинских организаций. Для расчетов и анализа использовали данные лабораторных исследований за 12 мес. Каждый участник исследования выбрал аналиты из перечня, предложенного ГОСТ Р 53079.3-2008, и рекомендованные этим стандартом критические уровни. Сложность и длительность доставки оценивали в условных единицах (от 1 до 5+) от наиболее простого способа доставки проб в лабораторию в течение максимум 1 ч после взятия крови в пределах одного здания до самого сложного, включающего большое число разноудаленных контрагентов, наличие курьерской службы вплоть до железнодорожной перевозки проб. Результаты. Малое число участников исследования не позволяет делать сколько-нибудь доказательные выводы, но весьма определенные комментарии и предположения, как нам кажется, сделать можно и должно, особенно учитывая дефицит публикаций на эту тему. Обращает на себя внимание 30-кратная разница доли пациентов с КЗ в лабораториях, преимущественно обслуживающих стационар, и в лабораториях, деятельность которых ориентирована в основном на амбулаторных пациентов. Трудности интерпретации -это отсутствие полноценной возможности оценивать уровень аналитического качества в этой зоне концентраций для большинства аналитов, что серьезно влияет на уверенную интерпретацию результатов. В списке рассмотренных аналитов были как минимум два преаналитически зависимых аналита -высокая концентрация калия и низкая концентрация глюкозы в крови. Очевидно, длительная транспортировка даже в пробирках с разделительным гелем не препятствует попаданию калия в сыворотку из форменных элементов крови. С критически низкой концентрацией глюкозы ситуация выглядит много лучше. При использовании пробирок с ингибиторами гликолиза большой разницы в доле КЗ по низкой глюкозе между лабораториями стационаров и крупных лабораторий со сложной системой доставки проб не обнаружено. Но для нас очевидно, что чем сложнее доставка проб, тем сложнее интерпретировать КЗ преаналитически зависимых аналитов. Aim of study: to give a comparative epidemiological characteristic of hepatitis C in Mongolia and Irkutsk region taking into account races, INF λ3 gene polymorphism and INF λ4 gene polymorphismin studied population. Epidemiological evidence of hepatitis C in Mongolia, in contrast to the Irkutsk region, characterized higher prevalence, domination of seropositive persons in age older than 50 years, domination in the circulation of the 1st virus genotype. In the population of Mongolia was revealed high frequency some nucleotide INF λ3 and INF λ4 gene polymorphism. These genetic markers are more favorable for natural history of hepatitis C. It explains the high rate of spontaneous hepatitis C virus clearance among people of Asian race.
Aim. Present comparative epidemiologic characteristics of viral hepatitis C in Mongolia and Irkutsk Region taking into account racial origin of the studied populations. Materials and methods. The studies were carried out in 2009 - 2014 on the territory of Irkutsk Region and Mongolia. Prevalence of viral hepatitis C based on serological monitoring, virus RNA detection, risk factors, change in structure of circulating genotypes, hepatocellular carcinoma morbidity were studied. Results. Epidemiologic manifestations of viral hepatitis C in Mongolia, in contrast to Irkutsk Region, are characterized by a wider prevalence of the disease, predominance of the fraction of seropositive individuals in age category of above 50 years and predominance of genotype 1 virus in circulation. In recent years an evolution of diversity of circulating virus genotypes took place towards a reduction of the fraction of genotype 1 in Mongolia and Russia due to an increase of the fraction of genotype 3. Expressed differences in average-annual values of hepatocellular carcinoma morbidity were detected, that were more than 10 times higher among Mongoloids compared with Caucasians. Conclusion. Pronounced differences were detected in manifestations of epidemic process of viral hepatitis C in Mongolia and Asian part of Russia, represented by Eastern Siberia, that are associated with ethnic, social and cultural living conditions of the indigenous population.
