Цель. Сравнение лечения коронавирусной инфекции у пациентов с сердечно-сосудистыми заболеваниями (ССЗ) в городских поликлиниках и инфекционном отделении госпиталя в реальной клинической практике. Материалы и методы. Исследование включало 88 пациентов с заболеваниями сердечно-сосудистой системы, госпитализированных по поводу коронавирусной инфекции, и 54 респондента поликлиник, получавших лечение амбулаторно. Проводился сбор анамнеза, опрос относительно принимаемой терапии и следования врачебным рекомендациям, а также о применении базисной терапии ССЗ. Результаты. В поликлиниках и стационаре часто назначались антибактериальные препараты (92,6% vs 100%) и их комбинации. Из 85,1% респондентов поликлиник только 44,4% принимали противовирусные препараты, входящие в перечень рекомендованных. 59,2% пациентов получали антикоагулянтную терапию, предпочтение отдавалось оральным антикоагулянтам. Треть наблюдаемых получала глюкортикостероиды (ГКС). В 70,3% случаев назначались различные витамины. В стационаре в качестве противовирусных препаратов 85% пациентов получали умифеновир, а 15% –интерферон альфа. При назначении антикоагулянтов предпочтение отдавалось нефракционированному (НФГ) и низкомолекулярному гепарину (НМГ). 77,3% пациентов получали нестероидные противовоспалительные препараты (НПВП) по показаниям. 88,6% респондентов были назначены ГКС. На неинвазивной респираторной поддержке находилось 45,5% пациентов. Выводы. Терапия COVID-19 на стационарном этапе была приближена к клиническим рекомендациям 9-го пересмотра, в отличие от лечения пациентов в поликлиниках. В стационаре все пациенты получали противовирусные препараты, рекомендованные Министерством здравоохранения. В большинстве случаев обоснованным было применение у них антикоагулянтной терапии и нестероидных противовоспалительных препаратов. В поликлинике часто использовались не рекомендованные Министерством здравоохранения противовирусные средства, пациентам с легкой формой COVID-19 часто назначались глюкокортикостероиды. Назначение антибактериальных препаратов среди респондентов как стационара, так и поликлиник было избыточным. Purpose. To compare the treatment of coronavirus infection in patients with cardiovascular diseases (СVD) in city polyclinics and infectious in-patient department of the hospital in real clinical practice. Materials and methods. The study included 88 cardiovascular patients hospitalized for coronavirus infection and 54 polyclinic respondents treated as out-patients. Patients’ histories were collected, and questioning about the therapy taken and compliance with medical recommendations, as well as about the use of CVD baseline therapy was conducted. Results. In both out-patient and in-patient clinics, antibacterial drugs and their combinations were often prescribed (92.6% vs. 100%, respectively). Of 85.1% of out-patient clinic respondents, only 44.4% received antiviral drugs included in the list of recommended medications. 59.2% of patients received anticoagulant therapy, with preference given to oral anticoagulants. One third of those observed received glucocorticosteroids (GCS). Various vitamins were prescribed in 70.3% of cases. In in-patient setting, 85% of patients received umifenovir and 15% received interferon alfa as antiviral medications. When prescribing anticoagulants, preference was given to unfractionated and low molecular weight heparin. 77.3% of patients received nonsteroidal anti-inflammatory drugs as per indications. 88.6% of respondents were prescribed glucocorticosteroids. 45.5% of patients underwent noninvasive respiratory support. Conclusions. COVID-19 therapy at in-patient stage was closer to the clinical guidelines in the 9th revision, in contrast to the treatment of patients in out-patient clinics. In the hospital, all patients received antiviral drugs recommended by the Ministry of Health. In most cases, their use of anticoagulant therapy and nonsteroidal anti-inflammatory drugs was justified. In the out-patient clinic, antiviral medications not recommended by the Ministry of Health were often used, and patients with mild COVID-19 were often prescribed glucocorticosteroids. Prescription of antibacterial drugs among both in-patient and out- patient clinic respondents was excessive.
Введение. Коронавирусная инфекция, вызываемая вирусом SARS-CoV-2, поразила весь мир. Число зарегистрированных случаев заболевания превысило 98,2 млн, а умерло 2,1 млн человек. Цель. Выявление у пациентов с сердечно-сосудистыми заболеваниями, госпитализированных по поводу коронавирусной инфекции COVID-19 легкой и среднетяжелой степени, особенностей клинических, лабораторно-инструментальных и психологических проявлений заболевания. Материалы и методы. Исследование включало 88 пациентов с заболеваниями сердечно-сосудистой системы, госпитализированных в инфекционное отделение по поводу коронавирусной инфекции. Всем пациентам проводились: сбор анамнеза, опрос относительно клинической картины заболевания, физикальный осмотр, лабораторно-инструментальная диагностика, заполнение шкалы HADS, шкалы оценки тревоги Бека, шкалы оценки депрессии Гамильтона. Результаты . Абсолютное большинство изученных пациентов относились к средней и старшей возрастной группе и имели среднетяжелое течение коронавирусной инфекции. Среди обилия симптомов со стороны различных систем органов преобладают клинические проявления поражения нервной системы: чувство страха и тревожности, нарушение сна, снижение памяти, заторможенность. У 47,7% пациентов выявлялось двустороннее полисегментарное поражение легких, соответствующее картине РГ-1. Также отмечались повышение уровня трансаминаз (>70%), гипергликемия (75%), ускорение СОЭ и повышение уровня СРБ (68,2%). При назначении лечения на амбулаторном и госпитальном этапе в 100% случаев избыточно назначалась антибактериальная и гормональная терапия. Заключение . Выявлено разнообразие клинических проявлений коронавирусной инфекции со стороны всех систем органов, в особенности нервной системы, в том числе изменения психической сферы пациентов в виде тревоги и нарушения когнитивных функций. В большинстве случаев выявляются: двустороннее поражение легких, гипергликемия, значительное повышение уровня трансаминаз и СРБ. Назначение антибактериальной и гормональной терапии в ходе лечения превышает реальную необходимость. Introduction . The coronavirus infection caused by the SARS-CoV-2 virus has hit the whole world. The number of reported cases exceeded 98.2 million and 2.1 million people died. Purpose. To identify the features of clinical, laboratory-instrumental and psychological manifestations of the disease in patients with cardiovascular diseases hospitalized for mild and moderate-severe COVID-19 coronavirus infection. Materials and methods. The study included 88 patients with cardiovascular diseases who hospitalized in the infectious diseases department for coronavirus infection. All patients underwent the following diagnostic methods: history taking, detailed questioning regarding the clinical picture of the disease, physical examination, laboratory and instrumental diagnostics, filling out the hospital anxiety and depression scale HADS, Beck’s anxiety rating scale, Hamilton’s depression rating scale. Results . The absolute majority of the studied patients belonged to the middle and older age group and had a moderate-severe course of coronavirus infection. Among the abundance of symptoms from various organ systems, clinical manifestations of nervous system damage predominate: feelings of fear and anxiety sleep disorders, memory loss, and lethargy. In 47.7% of patients, bilateral polysegmental lung damage was detected, corresponding to the picture of RG-1. There was also an increase in transaminase levels (>70%), hyperglycemia (75%), accelerated ESR, and an increase in CRP (68.2%). When prescribing treatment at the outpatient and hospital stages, in 100% of cases, antibacterial and hormonal therapy was excessively prescribed. Conclusion . A variety of clinical manifestations of coronavirus infection on the part of all organ systems, especially the nervous system, including changes in the mental sphere of patients in the form of anxiety and impaired cognitive functions, were revealed. In most cases, there are: bilateral lung damage, hyperglycemia, a significant increase in the level of transaminases and CRP. The appointment of antibacterial and hormonal therapy during treatment exceeds the real need.
